我院肿瘤药房静脉配置中心不合理医嘱分析

摘　　要：目的:为促进抗肿瘤药物临床使用的安全、有效提供参考。方法:利用PASS系统对我院肿瘤大楼2009年8月–2010年10月在院病人针剂医嘱进行人工审核,对不合理用药现象进行分析和总结。结果:我院肿瘤大楼各病区用药基本合理,不合理用药主要表现在溶媒不当(33.58%)、用药量过大或浓度过高(22.51%)、药物配伍禁忌(10.33%)、用药顺序不合理(8.86%)、给药途径错误(2.21%)、操作失误(4.43%)以及其他方面(18.08%)。结论:通过定期分析、总结,以药师提示单的形式及不合理医嘱的汇总形式与病房沟通,可以加强对药物配伍的监控,有效保障用药安全。Objective： To promote safety, efficacy of the antitumor drug use, and provide reference for clinic. Methods： The prescriptions of injection from Aug. 2009 to Oct. 2010 in our hospital were artificially audited and the irrational drug use was analyzed by using PASS system. Results： The drug use in tumor pharmacy of our hospital was basically reasonable, and the irrational drug use was mainly found in the improper solvent （33.58%）, high dosage or concentration （22.51%）, drug incompatibility （10.33%）, the improper order of drug use （8.86%）, the improper route of administration （2.21%）, operation error （4.43%） and other problems（18.08%）, etc. Conclusion： The communication with ward by pharmacists can strengthen the monitoring of drug compatibility and improve safe drug use.
