摘  要:目的探讨重组人生长激素(r—hGH)在中、重度慢性阻塞性肺疾病急性加重期(AECOPD)患者的营养状况、免疫功能、蛋白质代谢等方面的治疗作用。方法60例中、重度AECOPD患者随机分成两组,每组均给予抗感染、祛痰药、支气管扩张剂等常规治疗,对照组28例,标准的营养支持治疗;治疗组(r-hGH+常规治疗组)32例,在对照组常规治疗基础上,加用r-hGH。所有患者均于治疗前和治疗后第15天,测量肱三头肌皮褶(TSF)厚度、测定外周血血清白蛋白(ALB)、转铁蛋白(TRF)、前清蛋白(PA)、肌酐、尿素、免疫球蛋白(IgA、IgM、IgG)、采用流式细胞仪测定外周血T细胞亚群(CD3+、CD4+、CD4+/CD8+)比例的变化。结果治疗组与对照组相比,营养状况指标TSF厚度、ALB、TRF、PA均明显增加(P〈o.05);肌酐、尿素均明显减少(P〈O.05),免疫功能指标(免疫球蛋白IgA、IgM、IgG)、T细胞亚群(CD3+、CD4+、CD4+/CD8+)比例均明显增加(P〈0。05)。结论rhGH在改善中、重度AECOPD患者的营养状况、免疫功能和蛋白质代谢水平方面具有较好的作用。Objective To investigate the effects of recombinant human growth hor-mone(r-hGH) on the Nutritional status, immune function and protein metabolism in moderate and several AECOPD. Methods 28 patients with moderate and several AECOPD who received standard nutritional support were enrolled as control group. 32 patients with moderate and several AECOPD who received standard nutritional support and r-hGH were included as treatment group. All subjects received anti-infection therapy, phlegm-expelling formula, brochodilators. The levels of TSF, ALB, TRF, PA, ereatinine, urea, IgA, IgM,IgG were observed before and 15 days after treatment. Moreover, the variables including T-cell subsets, such as CD3 + , CD4 + , CD4 +/CD8 + . Results Compared with control group, subjects in treatment group had a significant increase in TSF, ALB, TRF, PA, creatinine, urea, IgA, IgM, and IgG( P 〈0.05 ). The T-cell subsets, such as CD3+, CD4+, CD4+/CD8+ were observably higher in treatment group. Conclusions r-hGH could induce better the Nutritional status,immune unction and protein metabolism in moderate and several AECOPD.
