桡骨远端粉碎性骨折及关节内骨折的内固定治疗

摘　　要：目的探讨桡骨远端骨折及关节内骨折的内固定治疗效果。方法回顾分析86例桡骨远端粉碎性骨折及关节内骨折患者进行"T"型锁定加压接骨板(LCP)手术方法治疗的临床资料,并与上年同期采用支架外固定相同病例资料对比分析。结果两组患者均随访6个月,骨折愈合平均为8周。观察组有3例,对照组有5例骨缺损大而行自体髂骨植骨或植入人工骨治疗。观察组总优良率82.56%,与对照组总优良率(81.40%)比较差异无统计学意义(P>0.05)。两组患者患侧腕关节活动度掌屈、背伸、旋前、旋后、桡偏、尺偏度差异无统计学意义(P>0.05)。两组患者并发症中,观察组无并发症,对照组骨折愈合延迟、钉道浅表感染各3例,拆除支架、口服抗生素后感染得到控制。对照组并发症发生率明显高于观察组,差异有统计学意义(P<0.05)。结论手术切开复位加LCP内固定治疗桡骨远端粉碎性骨折不需额外塑形,操作简单,保护了骨折块血运,利于骨折愈合。临床上应根据不同骨折类型选择相应的手术方法,以获得较好的疗效。Objective To evaluate the efficacy of internal fixation in the treatment of comminuted fracture of distal radial and intra-articular fracture.Methods 86 patients with comminuted fracture of distal radial and intraarticular fracture consisted of experimental group,the patients underwent locking compression plate and internal fixation.The patients underwent external fixation consisted of control group.Results The excellent rate was 82.56% and 81.40% in experiment and control group,the difference was not significant.The occurrence rate of complications was higher in experimental group than that in control group.Conclusion Surgical treatment of intraarticular fracture and comminuted fracture of distal radial by internal fixation can restore satisfactory function of wrist joint and restore the length of the radius relative to the ulna,articular surface,volar angulation and ulnar angulation atgreat range.
