摘  要:目的观察新生儿缺氧缺血性脑病(HIE)患儿单核细胞趋化蛋白-1(MCP-1)、肿瘤坏死因子-α(TNF-α)和可溶性细胞粘附分子-1(sICAM-1)的水平变化,探讨MCP-1、TNF-α和sICAM-1在判断新生儿缺氧缺血性脑损伤中临床价值。方法采用酶联免疫吸附试验(ELISA法)检测65例足月HIE患儿(轻度33例,中度19例,重度13例)及30例正常足月新生儿生后第1、7天血清MCP-1、TNF-α和sICAM-1水平。结果轻、中、重度HIE组患儿的MCP-1、TNF-α和sICAM-1水平较对照组显著升高(P<0.05);轻、中、重度HIE组MCP-1、TNF-α和sICAM-1水平逐渐升高,组间比较具有显著性差异(P<0.05);HIE患儿恢复期(生后7d)血清MCP-1、TNF-α和sICAM-1水平较急性期(生后1d)均有明显下降(P<0.05)。结论 MCP-1、TNF-α和sICAM-1可能在HIE的发病机制中起一定作用,检测血清中MCP-1、TNF-α和sICAM-1水平变化对判断HIE病情进展、指导治疗具有临床意义。Objective To study the level of serum MCP-1, TNF-αand sICAM-1 in neonatal with hy- poxic-ischemic encephalopathy (HIE) and to explore clinical value of serum MCP-1, TNF-αand sICAM-1 for brain damage on neonatal with HIE in judgment and prognosis. Methods The level of serum MCP-1,TNF-αand sICAM-1 in the 1st and 7th day after birth of the 65 cases term infant with HIE (33 cases were mild HIE, 19 cases were moderate HIE and 13 cases were severe HIE) were measured by Enzyme Linked Immunosorbent As- say(ELISA) ,and compared to 30 cases of normal newborns. Results The levels of serum MCP-1 ,TNF-αand sICAM-1 in mild, moderate and severe HIE groups were both significantly higher than those in controls (P 〈 0. 05 ). With increasing the degree of brain injury, the level of serum MCP-1, TNF-αand sICAM-1 increased( P 〈 0. 05 ). Compared to acute period ( the 1st day after birth), the level of serum MCP-1, TNF-αand sICAM-1 was clearly decreased in recovery stage(the 7th day after birth) in all HIE groups. Conclusion MCP-1 ,TNF-αand sICAM-1 may play an important role in the pathogenesis of neonatal HIE and be of value in our diagnosis and treatment.
