皮下蒂岛状鼻唇沟皮瓣血供研究及其修复鼻部分缺损的临床应用

摘　　要：目的探讨皮下蒂岛状鼻唇沟皮瓣血供特点,临床应用该皮瓣修复鼻部分缺损。方法 10例(20侧)成人尸体标本,经颈外动脉灌注红色乳胶,解剖观察鼻唇沟区及皮下蒂岛状鼻唇沟皮瓣的供血动脉。临床应用皮下蒂岛状鼻唇沟皮瓣修复鼻部分缺损75例(男55例,女20例),平均年龄34.5岁,皮瓣大小于2.5cm×1.5cm至5.5cm×3.0cm,观察术后皮瓣的成活及稳定性,外观及功能情况。结果鼻唇沟区血供主要来源于面动脉、眶下动脉、内眦动脉和面横动脉及其分支,上述动脉分支入浅筋膜形成皮下动脉网,再由此分支至真皮下层形成真皮下动脉网以供应该区皮肤,皮下蒂岛状皮瓣血供来自皮瓣区皮下动脉网,蒂部供血源自上述任一血管的分支血管。75例皮瓣均成活良好,术后随诊外形、功能满意。结论皮下蒂岛状鼻唇沟皮瓣供血主要来源于面动脉,眶下动脉、内眦动脉和面横动脉的分支相互吻合形成之皮下血管网,皮下蒂岛状鼻唇沟皮瓣可以由上述任一血管的分支供血。皮下蒂鼻唇沟皮瓣为修复鼻部分缺损提供了一个简单、可靠、易操作,修复效果良好的方法。Objective To study the arterial anatomy of the subcutaneous island pedicle nasolabial flap and its clinical application in the reconstruction of nasal partial defects. Methods The microanatomy of blood vessels of nasolabial groove region was investigated using 10 adult cadavers. The clinical study included 75 patients with a mean age of 34,5 years.The size of the flap varied from 2.5 crux 1.5cm to 5.5 cm x3.0cm. Results The blood supply of the subcutaneous island pedicle nasolabial flap depends basically on the subdermal arterial network that formed by the perforator branches of the facial artery, infraorbital artery, angular artery, transverse facial artery. The blood supply of the pedicle originate from the perforator branches of any one of these arteries. All flaps survived completely, the aesthetic and functional results of each case were satisfactory. Conclusion The blood supply of the subcutaneous island pedicle nasolabial flap depends basically on the subdermal arterial nwork that formed by the perforator branches of the facial artery, infraorbital artery, angular artery and transverse facial artery, the blood supply of the pedicle originated from the perforator branches of any one of these arteries. The subcutaneous island pedicle nasolabial flap is simple, dependable and easy to harvest flap that can repair the nasal partial defects.
