集束干预方案预防术中压疮的研究

摘　　要：目的:探讨集束干预方案在术中压疮预防中的应用效果。方法:集合有循证基础的护理措施,参考对手术室护士术中压疮的问卷调查和技能考核结果,制定术中压疮六步防护策略,并进行为期三个月的相关知识、集束干预方案培训与督导。选择集束干预方案实施后的800例手术患者为实验组,实施前的800例手术患者为对照组进行效果比较,并对78名手术室护士再次进行问卷调查。结果:78名护士的知信度分值和技能考核成绩在集束干预方案实施前后差异有统计学意义(P<0.01);实验组术中压疮发生率为1.6%,显著低于对照组;不同手术体位及手术时间压疮发生率差异具有统计学意义。结论:制定并应用术中压疮集束干预方案,通过持续教育及过程管理,可提高护士的压疮防护意识及能力,有效降低术中压疮的发生率。Objectives： To explore the effect of intervention bundle on intraoperative pressure sores prevention. Methods： We collected the data of evidence-based intervention and questionnaire and skills assessment results about intraoperative pressure ulcers among nurses in operating room. We developed a six-step intraoperative pressure sores care strategies, trained the nurses with related knowledge and skills, supervised their work, and completed the intervention bundle program in three months. We recruited 800 surgical patients in the experimental group and 800 surgical patients before intervention bundle program in the control group. We conducted a questionnaire survey and skills assessment again among 78 operating room nurses. Results： The knowledge, belief and skills scores of nurses were improved significantly after intervention （P〈0.01）.The incidence of pressure sores was 1.6% in the experimental group which was significantly lower than the control group. There were significant differences of the incidence of pressure sores among patients in different operative position and operative time. Conclusion： Intervention bundle program could apply in intraoperative pressure sores prevention, provide continuous education and process management, improve the nurses protection awareness and competence, and decrease the incidence of intraoperative pressure sores.
