摘  要:目的探讨唾液酸酶联合检测在阴道炎中的应用价值和意义。方法回顾性分析我院2010年12月~2011年5月收治的538例阴道炎患者的临床资料,所有患者均进行阴道分泌物白带三项检测[过氧化氢(H2O2)浓度、唾液酸酶、白细胞酯酶1,快速诊断阴道炎,并与传统的Amsel法检出结果比较。结果以Amsel法为标准,H2O2联合唾液酸酶检测的符合率、敏感性、特异性分别96.1%、83.2%、98.9%,唾液酸酶单项检测的符合率、敏感性、特异性分别为94.4%、88.4%、95.7%,H2O2联合唾液酸酶检测对阴道炎的诊断优于唾液酸酶单独检测,差异有统计学意义(P〈0.05)。H2O2浓度异常与pH〉4.6检出有较好的一致性,白细胞酯酶阳性检出与清洁度〉Ⅲ度检出有较好的一致性。结论唾液酸酶联合检测对阴道炎的诊断有较高的临床价值,为提高诊断率,在临床检验时可结合阴道分泌物的清洁度、滴虫、霉菌检查作为常规检查项目。Objective To investigate the clinical application value and significance of sialidase joint detection in vaginitis. Methods Clinical data of 538 cases of patients with vaginitis in our hospital form December 2010 to May 2011 were analyzed retrospectively. All patients underwent vaginal secretions leucorrhea three items detection (concentration of hydrogen peroxide, sialidase, leukocyte esterase) and were rapid diagnosis of vaginitis, which were compared with results of the conventional Amsel method. Results Taken Amsel method as the standard, the coincidence rate, sensitivity, specificity of H2O2 combined with sialidase detection were 96.1%, 83.2%, 98.9%; the coincidence rate, sensitivity, specificity of sialidase single detection were 94.4%, 88.4%, 95.7% respectively; H2O2 combined with sialidase detection in the diagnosis of vaginitis was better than that of sialidase single detection (P 〈 0.05). The detection rate of abnormal concentration of H2O2 had better consistency with pH 〉 4.6 detection. Leukocyte esterase positive detection had better consistency with cleanliness 〉 III degree detection. Conclusion Sialidase joint detection has high clinical value in diagnosis of vaginitis. In clinical inspection it can be combined with vaginal cleanliness, trichomonas, mold examination as routine examination items, in order to improve the diagnosis rate.
