双腔起搏对三度房室传导阻滞病人左心房收缩功能的影响

摘　　要：目的评价三度房室传导阻滞病人植入双腔起搏器后对左心房收缩功能的影响。方法筛选20例三度房室传导阻滞病人,分别于双腔起搏器植入术前及术后1、3个月应用应变率成像(SRI)技术评价左心房的收缩功能。结果与术前比较,术后1个月左心房排空分数(LAEF)、左心房收缩期峰值应变率(SRa)、左心室射血分数(LVEF)和E/A显著升高;术后3个月,上述指标变化更为显著,差异均有统计学意义(F=6.798～25.763,P<0.05)。术后1个月和3个月LVEF变化率与SRa变化率呈正相关(r=0.500、0.525,P<0.01)。结论双腔起搏器植入术可改善三度房室传导阻滞病人左心房的收缩功能,表现为辅泵功能增加,同时改善病人的心功能。Objective To evaluate the effect of dual chamber cardiac pacing on left atrial （IRA） systolic function in pa- tients with third degree atrioventricular （A-V） block. Methods Twenty patient8 with third degree A-V block were enrolled, their LA systolic function was evaluated before and one and three months after pacemaker implantation by strain rate imaging （SRI）. Results Compared with the baseline, left atrial ejection fraction （LAEF）, the peaks of SR at atrial contraction （SRa）, left ventricular ejection fraction （LVEF） and E/A increased at one month, at three months, the above changes were more signifi- cant （F=6. 798-25. 763 ,P〈0. 05）. The changes of LVEF were positively correlated with SRa at one and three months after im plantation of pacemaker （r=0. 500,0. 525;P^0.05）. Conclusion The implantaion of dual-chamber pacemaker can improve the cardiac function of patients with third degree A V block, manifesting an increase of pump function.
