摘 要:目的探讨新生儿重症监护室(NICU)中足月小样儿听力障碍发生情况,以便早期诊断、干预。方法采用瞬态诱发耳声发射(TEOAE)方法,对2009年1月—2010年12月期间在我院NICU住院的120例足月小样儿生后3~5d进行初筛,未通过者30~42d进行复筛,仍未通过者3月龄时行听性脑干反应(ABR)检查。所得结果与150例正常新生儿进行比较。结果足月小样儿听力筛查初筛未通过率53.33%,复筛未通过率15.79%,ABR检查未通过率7.14%,均明显高于对照组(初筛未通过率9.33%,复筛未通过率2.01%,ABR未通过率0.67%),差异均有显著性(χ2=8.04~63.83,P<0.01)。结论足月小样儿听力障碍发生率明显高于足月正常新生儿,应常规筛查,加强宣教,早期干预。Objective To investigate hearing disturbance in full term fants with low birth weight (LBW) in neonatal intensive care unit (NICU), so as to make an early diagnosis and intervention. Methods Using transient evoked otoacoustic emission (TEOAE) method, 120 term inlands with LBW treated in NICU underwent a preliminary screening on 3-5 days after birth, for those who failed to pass the screening, a reexamination was done on clays 30-42, and if failed again, auditory brain stem re- sponse (ABR) was done at three months old. The result was compared with 150 normal infants. Results In preliminary screening, 53.33%of the LBW infants failed to pass the examination; in re examination,15.79% failed; in ABR, 7.14% failed versus 9.33%, 2. 01% and 0. 67%, respectively, in normal infants, the differences between the two groups were significant (x^2= 8. 04-63.83,P〈0. 01). Conclusion The incidence rate of hearing disturbance is higher in full term infants with LBW than normal infants, a routine screening, instruction and early intervention should be conducted.
