摘  要:目的探讨距下关节横切口隧道置入螺栓加压钢板治疗跟骨骨折的临床效果。方法 2011年1月—2011年4月应用距下关节外侧横切口入路,解剖型螺栓加压钢板内固定治疗累及距下关节的跟骨关节内骨折26例30足,其中SandersⅡ型10足,Ⅲ型15足,Ⅳ型5足。所有患者均获得随访,随访时间6~12个月,平均8.8个月。结果术后无切口感染、血管神经损伤、内固定松动、腓骨长短肌腱炎等并发症,2例SandersⅣ型患者出现距下关节疼痛,经X线片证实有距下关节创伤性关节炎。跟骨长度、高度、宽度、Bohlers角、Gissan's角恢复满意。应用Maryland跟骨骨折功能评分进行评价,其中优19例,良6例,可1例,优良率96.2%。结论经距下关节横切口隧道置入螺栓加压钢板治疗跟骨关节内骨折创伤小、并发症少、临床效果满意。Objective To explore the clinic outcome of compression fixation with steel plate, stud bolt and transverse incision through subtalar joint for calcaneal fractures. Methods From January 2011 to April 2011,30 feet in 26 patients with calcaneal fractures were treated, transverse incision through subtalar joint and anatomical compression plate and stud bolt were used. According to Sanders classification system,there were 10 feet of type II ,15 feet of type m and 5 feet of type IV. All cases were followed up for 6 to 12 months with an average of 8.8 months. Results There were no operative complications including infection of incisional wound, damage of blood vessel and nerve, looseness of fixation, myotenositis of long and short muscle tendon of fibula. Two cases with Sanders IV emerged joint traumatic subtalar arthritis with pain of subtalar joint, which were confirmed traumatic arthritis by X - ray. The height,length,width, Bohler's and Gissan's angles of calcaneus were almost restored. The results were evaluated using Maryland Food Score. Excellent results were noted in 19 cases, good in 6, fair in 1, and the excellent and good rate were 96.2%. Conclusion Compression fixation with steel plate, stud bolt and transverse incision through subtalar joint for calcaneal fractures exhibits minimal invasion, few" complications and favorable clinical results.
