胰胆管合流异常与胰腺炎关系的探讨

摘　　要：探讨胰胆管合流异常 (AJPBD)与胰腺炎的关系。方法　回顾性分析胆道造影、胰腺组织学、超微结构检查以及血清、胆汁淀粉酶的测定结果。结果　可分析对象 13 68例中发现AJPBD 3 7例 ,发生胰腺炎 2 8例 ,其中急性胰腺炎 6例 ,慢性胰腺炎 2 2例。光镜下胰腺均有炎性细胞浸润和 或纤维组织增生 ,其超微结构均有不同程度的病理改变。AJPBD组血清和胆汁淀粉酶及胰腺炎的发病率均高于对照组 (P <0 .0 5)。结论　AJPBD与胰腺炎有密切关系 ;Objective To study the relationship between anomalous junction of pancreaticobiliary duct(AJPBD) and pancreatitis. Methods The data of cholangiography, pancreatic histology and ultrastructure examination, serum and bile amylase measurement were analysed retrospectively. Results Of 1,368 patients, 37 patients with AJPBD were found. There were 28 patients with pancreatitis in the 37 patients with AJPBD; of them 6 with acute pancreatitis and 22 chronic pancreatitis. Microscopically, there were cells infiltration and/or fibrosis in the pancreas. Electromicroscopic examination showed various degrees of ultrastructural pathological changes. The serum and bile amylase levels and incidence of pancreatitis in the AJPBD patients were higher than those in control group( P <0.05). Conclusions AJPBD are closely associated with pancreatitis. The pancreatitis caused by AJPBD evolves slowly.
