摘  要:目的研发医院感染暴发实时监测系统,对感染暴发进行及时灵敏的预警。方法依据医院感染暴发的定义和科室感染日常监测数据,制定出科室感染暴发阈值,提出医院感染暴发的筛查策略和预警展示条件。结果系统能实时监控医院感染暴发的高危因素及暴发流行风险,达到有效暴发预警效果;感染暴发不仅包含确诊的感染病例,也进行住院病例临床症状的暴发监测,实现暴发预警进一步"关口前移";应用监测系统近两年时间,预警疑似暴发>10起,并进行临床调查及采取相应措施,成功地避免多次医院感染的暴发。结论医院感染暴发监测系统实现了感染暴发的有效预警,具有实时、高效、灵敏的特点。OBJECTIVE To establish the real-time monitoring system of nosocomial infections in order to predict the outbreak of nosocomial infections in the early stage.METHODS The outbreak threshold was formulated according to diagnostic criteria of outbreaks and daily monitoring data,then the screening strategy and prewarning condition of outbreak were instituted by referring to the daily monitoring and the threshold.RESULTS The system could monitor the high risk factors for the nosocomial infections as well as the risk of prevalence,meeting the need of outbreak predication.The outbreak surveillance not only included the definite infectious cases but the suspected clinical infectious symptoms;these made the surveillance more sensitive,which could be called 'check-point ante displacement';after nearly 2 years of applying the outbreak surveillance system,more than 10 pieces of suspected outbreaks were predicted,the clinical investigation was performed and the corresponding measures were adopted,many cases of nosocomial infections being successfully avoided.CONCLUSION The real-time monitoring system of nosocomial infections,which is real-time,highly effective and sensitive,achieves the effective early warning of nosocomial infections.
