活血祛风通络法对类风湿关节炎患者指腹微循环血流的影响

摘　　要：目的:观察活血祛风通络法对类风湿关节炎患者治疗前后血液微循环指标变化的影响,探讨中医药在治疗RA过程中的作用机理。方法:将确诊的81例患者随机分为两组,对照组口服来氟米特片、洛索洛芬钠片治疗,治疗组在对照组用药的基础上,加服自拟活血祛风通络中药方剂。3个月为1个疗程。治疗前和1个疗程后分别应用增强型能量多普勒显像观察两组类风湿关节炎患者的手指指腹动脉的收缩期峰值速度(PSV)、舒张期峰值速度(EDV)以及血管阻力(RI)参数、血液流变学相关指标和临床症状的改变,并进行疗效评定。结果:①治疗组的总有效率为90.24%,明显高于对照组的75.00%(P<0.05)。②治疗组的临床症状及体征与对照组比较改善程度更明显。③治疗组的指腹微循环指标PSV、EDV、RI及血液流变学相关指标较对照组均有明显改善。结论:活血祛风通络法联合来氟米特片、洛索洛芬钠片能改善类风湿关节炎患者微循环障碍,治疗类风湿关节炎疗效肯定,且较单纯西医治疗更具优势。Objective：Observe before and after treatment of promoting blood circulation,dispelling wind and removing obstruction in patients with rheumatoid arthritis index changes of blood circulation,and explore the process of Chinese medicine in the treatment of RA in the mechanism.Methods：The diagnosis of 81 patients were randomly divided into 2 groups,which the control group patients taking oral leflunomide tablets and loxoprofen sodium tablets,while the control group patients on the basis of medication,plus taking prescription chinese medicine from the proposed treatment of promoting blood circulation,dispelling wind and removing obstruction.3 months as a course of treatment.After a course,application of enhanced power Doppler imaging were observed in both groups before and after treatment in patients with rheumatoid arthritis finger pulp artery peak systolic velocity（PSV）,diastolic peak velocity（EDV） and vascular resistance（RI） parameters,hemorheology related indicators and changes in clinical symptoms.Then did therapeutic evaluation after the observation.Results：①The total effective rate in treatment group is 90.24%,significantly higher than the 75.00% of control group（P〈0.05）.②Compared with the control group,the clinical symptoms and signs of treatment group improved more significantly.③The treatment group pulp microcirculation index PSV,EDV,RI and hemorheology compared with the control group showed significant improvement.Conclusion：The promoting blood circulation,dispelling wind and removing obstruction method combined with leflunomide tablets,loxoprofen sodium tablets in the treatment of rheumatoid arthritis microcirculation is effective,and is more competitive than Western medicine alone.
