芒针循经透刺对颈源性眩晕镇眩作用及即时效应观察  

Clinical Observation on the Instant-Effect and Antivertigo Function of Penetration Acupuncture Along Channel with Mang Needling on Cervical Vertigo

摘　　要：目的:观察芒针循经透刺对颈源性眩晕镇眩作用及即时效应。方法:将234例病人随机分为实验组(118例)和对照组(116例)。实验组用芒针循经透刺治疗,对照组用静滴麦普宁针治疗。观察第1次针刺治疗前后即时经颅多普勒(TCD)检查左右两侧椎动脉(VA)和基底动脉(BA)的收缩峰期血流速度(Vpeak),舒张末期血流速度(Vmin)及血管搏动指数(PI)、血管阻力指数(RI)值,并分别于治疗后1、2、3、4……7天对眩晕程度进行评分,最后进行统计学处理。结果:两组镇眩效果:实验组显效112例(86.44%);对照组显效71例(61.20%);两组综合疗效:实验组:治愈93例(78.81%);对照组:治愈67例(57.76%)。经统计学处理,有显著性差异(P<0.01)。即时效应:经针刺治疗后,VA及BA的Vpeak和Vmin值明显增加(P<0.01),恢复或接近正常水平。VA、BA的PI和RI值明显下降(P<0.01)。结论:芒针循经透刺能使颈源性眩晕患者椎—基底动脉血管舒缩弹性好转,明显改善脑部血供,从而达到镇眩作用。Objective：To observe the clinical instant-effect and antivertigo function of penetration acupuncture along channel with Mang Needling on cervical vertigo.Methods：234 cases of cervical vertigo were randomly divided into experimented group（n=118）and controlled group（n=116）.The experimented group was treated with Mang Needling penetration acupuncture along channel and the controlled group with Puerarin for Injection.Transcranial Doppler（TCD）indices of vertebral artery（VA）and basilar artery（BA）,such as peak systolic velocity（Vpeak）,end diastolic velocity（Vmin）,pulsatility index（PI）and resistance index（RI）were observed before and after the first treatment with acupuncture.Then the score of vertigo degree after compressing was evaluated respectively 1,2,3,4……7d after treatment and made statistic analyse.Results：Antivertigo function in the two groups：in the experimental group,remarkable effect rate was 86.44%（112 cases）;in the controlled group,remarkable effect rate was 61.20%（71 cases）.Comprehensive function in the two groups：in the experimental group,the cure rate was 78.81%（93 cases）;in the controlled group,the cure rate was 57.76%（67 cases）,indicating significant difference（P〈0.01）.Instant-effect：after the first treatment with acupuncture,the Vpeak and Vmin were significantly elevated to or near to normal level,while PI and RI dropped significantly（P〈0.01）.Conclusion：The Mang Needling penetration acupuncture along channel could make the blood vessel of vertebralis-basilaris arteriae elasticity better,the blood supply of the brain could be improved after treatment with penetration acupuncture along channel,and it bring a definite antivertigo effect for cervical vertigo.
