摘 要:目的:通过观察针刺对实验性糖尿病性大鼠心肌组织趋化因子-1(MCP-1)表达影响的研究,进一步探讨针刺在治疗糖尿病性心肌病(DCM)方面的作用和效果。方法:选用48只健康Wistar雄性大鼠按体重随机分为对照组8只,实验性糖尿病(DM)模型制作组40只,通过腹腔注射链脲佐菌素(STZ),诱导DM大鼠模型。将确定为DM的34只大鼠随机分为3组,即模型组11只、依那普利治疗组11只、针刺治疗组12只。针刺治疗组选肺俞、脾俞、肾俞、三阴交、内关、胃管下俞,每日针刺1次,每次15min,连续针刺12周;依那普利治疗组每日灌服依那普利1.5mg/kg。检测大鼠空腹血糖水平;对大鼠体重进行前后对比;14周末,HE染色光镜下观察心肌组织;免疫组化染色观察心肌组织MCP-1蛋白的表达水平。结果:治疗后,针刺组大鼠FBG较治疗前显著下降(P<0.01),明显改善DM大鼠的一般状态;针刺组和依那普利药物治疗对照组的MCP-1在心肌内表达从染色强度到面积比DCM模型组均明显减轻,半定量分析结果表明,针刺组的MCP-1蛋白表达接近于对照组,针刺组与依那普利阳性对照组心肌组织单核巨噬细胞浸润明显降低,分别与DCM模型组比较,均具有显著性统计学意义(P<0.01),两组之间比较,针刺组优于依那普利阳性对照组。结论:针刺可明显改善实验性DCM大鼠心肌病理组织,对DCM心肌具有一定的保护作用,较明显的抑制DCM大鼠心肌MCP-1蛋白表达的作用。心肌组织MCP-1蛋白参与了DCM发病过程,提示针刺防治DCM的作用可能是通过抑制心肌组织MCP-1途径,减少MCP-1过度表达,抑制DCM炎症发病机制有关。Objective:In order to further explore the role and effects of acupuncture in the treatment of diabetic cardiomyopathy(DCM)through observation of acupuncture on MCP-1 expression of experimental diabetic rat myocardium.Methods:The 48 male Wistar rats were randomly divided into control group(8 rats)and the remaining 40 were given single intraperitoneal injection to make diabetic cardiomyopathy rat model.34 successful model rats were randomly divided into model group(11 rats),western medicine(Enalapril)group(11 rats),and acupuncture group(12 rats).Acupuncture group selected Feishu(BL13),Pishu(BL20),Shenshu(BL23),Sanyinjiao(SP6),Neiguan(PC6),Weiwanxiashu(EX-B3),once a day,15mins each time for 12 weeks;everyday western medicine group rats were fed with Enalapril 1.5mg/kg.Detection of target:fasting blood glucose level testing;body weight in rats before and after comparison;after 14 weeks,HE stained myocardial tissue;immunohistochemical staining of myocardial tissue MCP-1 protein expression.Results:FGB of acupuncture group was significantly lower than that before treatment(P〈0.01),the general state of DM rats were improved.From the area ratio and staining intensity,MCP-1 expression in the myocardium of acupuncture group and western medicine(Enalapril)group decreased significantly compared with that in the DCM model group.Semi-quantitative analysis showed that MCP-1 protein expression of acupuncture group was close to that of the normal control group.Myocardial infiltration of monocyte-macrophage cells of acupuncture group and western medicine(Enalapril)group were significantly decreased,and both had significant statistical significance(P〈0.01)compared with that in the DCM model group;while from the comparison between the two groups,acupuncture group was better than western medicine(Enalapril)group.Conclusion:Acupuncture can improve the experimental DCM myocardium pathology,and has a protective effect on DCM myocardium and more pronounced i
