腹腔镜下肝叶切除术20例临床体会

摘　　要：目的探讨腹腔镜肝切除(LH)的手术指证和手术技巧。方法回顾性分析2009年1月～2011年2月对20例施行完全腹腔镜肝切除术患者手术时间、术中失血量及术后并发症等。结果该组19例手术在完全腹腔镜下完成,1例分离第二肝门时损伤肝右静脉而中转开腹。手术时间110～400min,平均218min,术中出血量50～800mL,平均305mL。术后住院7～17d,平均9.5d。无围手术期死亡及严重并发症发生。结论合理掌握手术指证和技巧,可安全进行腹腔镜下肝叶切除术。[ Objective ] To explore the indication and the techniques of clinic application of laparoscopic hepate- ctomy （LH）. [Methods] The clinical data of 20 cases receiving the procedure in our hospital from January Novem- ber 2009 to February 2011 were retrospectively analyzed. [ Results ] Laparoscopic hepatectomy was performed suc- cessfully under laparoscope in 19 cases and converted to open operation in 1 because of intra-operative hemorrhage. In laparoscopic hepatectomy, the median operative time was 218 rain （110~400 rain）, the median blood loss was 305 mL （50-800 mL）. No perioperational death and critical postoperative surgical complications occurred. The median length of postoperative hospital stay was 9.5 d （7-17 d）. [Conclusion] Laparoscopic hepatectomy is safe and feasi- ble by using reasonable indication and the techniques.
