紫金锭外敷治疗冠状动脉造影和介入术后穿刺部位皮下瘀血的疗效观察

摘　　要：目的观察经股动脉穿刺行冠状动脉造影或冠状动脉介入治疗术后穿刺部位的皮下淤血,应用紫金锭外敷的疗效,探讨护理措施。方法选择2011年3月—2012年3月,经股动脉穿刺插管进行冠状动脉造影或经皮冠状动脉介入术后,在拔除冠状动脉穿刺点动脉鞘管后出现血肿的患者,即给予紫金锭药液湿敷治疗,观察血肿面积及消退、疼痛、体温等情况。结果敷药前与敷药的第1天,第2天,第3天的血肿面积比较,差异有统计学意义(P<0.01),即血肿消退效果第3天优于第2天,第2天优于第1天。VAS疼痛判断,0～3分占83.4%,4～6分占16.6%。体温36.3～36.45℃,无发热及炎症反应。结论应用紫金锭外敷冠状动动脉造影和介入术后穿刺部位皮下淤血的治疗操作简单、疗效好。Objective To observe the efficacy of topical applications of purple tablets on the patients with subcutaneous ecchymosis at the puncture site after Coronary Angiography or Percutaneous Coronary Intervention via the femoral arterial puncture and explore nursing interventions. Methods The patients who accepted Coronary Angiography or Percutaneous Coronary In- tervention via the femoral arterial puncture were collected from March 2011 to March 2012. When postoperative hematoma at the puncture site appeared after the removal of arterial sheath at the puncture site, put a wet compress with purple tablets immediate- ly. And then observed the variation of hematoma area, pain and temperature of the patients. Results Compared with before treated , the hematoma area after treated one day, two days and three days were statistically significant （P 〈0. 01 ）, The hema- toma subsided effect ： three days was better than two days, two days was better than one day. VAS pain judgment ： 0 - 3 points accounted for 83.4%, 4 -6points accounted for 16. 6%. Body temperature Was between 36. 3 to 36.45℃, No fever and inflam- mation was observed. Conclusion Using purple tablets at the puncture site after Coronary Angiography or Percutaneous Coro- nary Intervention to treat subcutaneous ecchymosis should be introduced for its simple operation as well as good efficacy.
