睾丸扭转45例诊治体会

摘　　要：目的对睾丸扭转的诊断与治疗方法进行总结分析,提高睾丸扭转的诊治水平。方法回顾性分析2007年1月～2010年12月期间本院收治的45例睾丸扭转患者的临床资料,对其诊断和治疗过程进行总结分析。结果 40例经彩色多普勒超声检查确诊,5例经手术探查确诊;45例患者均予手术治疗,术中见睾丸扭转角度在180°～550°,32例给予睾丸复位后保留固定治疗,13例给予睾丸切除术治疗,随访观察1年,出现睾丸萎缩3例。结论彩色多普勒超声检查并诊断睾丸扭转的敏感性和特异性较高,大部分可明确诊断,难以确诊者可予手术探查,扭转时间短者可予保留睾丸并复位固定治疗,扭转时间较长者应视患者情况予手术切除睾丸,一般预后良好。Objective To summarize the methods of the diagnosis and treatment of testicular torsion,and elevate the level of diagnosis and treatment. Methods The clinical data of 45 cases of testicular torsion who cured in our hospital during Jan- uary 2007 to December 2010 were retrospectively analyzed. Their diagnosis and treatment process was analyzed. Results The diagnosis of testicular torsion in 40 cases was based on color doppler ultrasonography. 5 cases were verified by opera- tion. 4＇5 cases were confirmed by surgery, in which 32 cases were salvaged by an orchidopexy,13 cases treated with or- chie^omy. The Angle of testicular torsion was between 180~to 550~. Followed-up for 1 year, 3 cases were testicular atro- phy. Conclusion The color Doppler ultrasonography were more sensitive and peculiar in the diagnosis of testicular torsion. Most of the patients could be clear diagnosised, the patients who were difficult to diagnose could be probed under surgery, The one who turned in the short time could be treated by keeping testicular and reset fixed treatment,and who turned for long should be given the surgical resection testes,The prognosis was generally good.
