摘 要:目的乳腺普查中应用带超声弹性成像(UE)与彩色多普勒血流成像(CDFI)对乳腺良恶性肿块进行鉴别诊断。方法回顾性分析276例乳腺患者共286个肿物,经术后病理确诊良恶性,对比分析两者检查的敏感性、特异性、准确性,绘制ROC曲线,计算AUC面积,比较两者差异。结果超声弹性成像在成像上和诊断的敏感性、特异性、准确性方面都高于彩色多普勒超声,差异有统计学意义(P<0.05),ROC曲线显示UE在诊断早期乳腺癌方面强于CDFI,AUC差异具有统计学意义(P>0.05)。结论带超声弹性成像的彩色多普勒超声在女性乳腺普查中有简易、重复、便利的效果,有利于发现早期乳腺癌,提高保乳率。Objective To use ultrasound elastography (UE) and color doppler flow imaging (CDFI) to identify benign or malignant breast tumor in breast census. Methods Retrospective analyses in 286 tumors from 276 breast patients are used. Analyse the sensitivity, specificity and accuracy, then draw the ROC curves and calculate AUC areas of benign and malig- nant identified by postoperative pathologic diagnosis comparatively to compare the difference. Results UE was superior to CDFI in both imaging and diagnosis of sensitivity, specificity and accuracy, the difference was statistically significant (P 〈 0.05). ROC curves show UE was superior to CDF in identifing early breast cancer, AUC difference was statistically signif- icant (P 〉 0.05 ). Conclusion Using Color Doppler with Ultrasound Elastography is a convenient and repeatable way in female breast census to indentify early breast cancer and improve the rate of breast.
