经右胸小切口心脏手术518例体会

摘　　要：目的探讨经右胸小切口行心脏手术的可行性。方法总结518例经右胸小切口心脏手术患者的临床资料,室间隔缺损(VSD)256例,房间隔缺损(ASD)229例,肺动脉狭窄(PS)18例,法洛四联症(TOF)7例,主动脉瓣替换(AVR)5例,二尖瓣替换(MVR)2例,左房黏液瘤(LAM)1例。结果全组无手术死亡病例,术后二次开胸止血2例,肺不张3例。结论经右胸小切口行心脏手术具有出血少、关胸快、美观,且不破坏胸廓的连续性,防止术后鸡胸等优点,是患者及家属乐意接受的一种手术方法。Objective This paper reported the experience of intracardiac surgery through right minithoracotomy incision. Methods 518 cases underwent intracardiac surgery through right minithoracotomy incision under extracorporeal circulation. 256 cases of ventricular septal defect, 229 cases of atrial septal defect, 18 cases of pulmonary stenosis, 7 cases of tetralogy of Follot, aortic valve replacement in 5 casas, mitral valve replacement in 2 cases, left atrial myxoma in 1 case. Results No operative mortality was observed in this study. R re-operation was needed for stopping bleeding in 2 cases after the operation, and pulmonary atelectasis were observed in 3 cases. Conclusion Intracardiac surgery through right minithoracotomy incision causes less bleeding, quick closure of thoracic incision, maintainsthe continuity and the integrity of the bony thorax, and prevents postoperative pigeon breast and good cosmetic results.
