摘 要:目的检测慢性乙型肝炎(CHB)患者外周血细胞因子白细胞介素(IL)-9、IL-10及转化生长因子-β1(TGF-β1)的水平,并探讨其临床意义。方法采用酶联免疫吸附试验(ELISA)检测98例CHB患者和20例健康体检者(对照组)外周血中细胞因子IL-9、IL-10及TGF-β1水平。结果 CHB轻中度组、CHB重度组及肝硬化组血清IL-9水平分别为(319.96±367.26)ng/L、(308.47±402.77)ng/L、(432.49±591.65)ng/L,IL-10水平分别为(95.73±107.93)ng/L、(69.52±66.49)ng/L、(111.41±113.41)ng/L,TGF-β1水平分别为(42.40±56.57)μg/L、(138.85±178.55)μg/L、(135.32±179.44)μg/L,均明显高于正常对照组[(139.52±193.44)ng/L、(29.73±21.27)ng/L、(15.30±14.51)μg/L,P均<0.05];血清IL-9、IL-10水平与不同HBV DNA载量有关,而IL-9、IL-10及TGF-β1水平与不同的HBeAg状态无关。CHB各组IL-9与IL-10呈强正相关(r=0.932,P<0.001),IL-9与TGF-β1呈弱正相关(r=0.227,P=0.014);IL-10与TGF-β1呈弱正相关(r=0.217,P=0.018)。结论 IL-9、IL-10及TGF-β1均参与CHB的发病过程,IL-9及IL-10可能具有抑制炎症作用,其持续升高可能与CHB患者慢性感染状态有关。Objective To measure the levels of cytokines IL-9,IL-10 and TGF-β1 in the peripheral blood of patients with chronic hepatitis B(CHB).Methods Blood samples from 98 CHB patients and 20 healthy controls were collected.Serum IL-9,IL-10 and TGF-β1 levels were determined by enzyme-linked immunosorbent assay(ELlSA).Results The IL-9 levels in the serum of slight/moderate,severe and liver cirrhosis CHB patients were(319.96±367.26),(308.47±402.77)and(432.49±591.65)ng/L,respectively;the IL-10 levels in these three CHB groups were(95.73±107.93),(69.52±66.49)and(111.41±113.41)ng/L,respectively;the TGF-β1 were(42.40±56.57),(138.85±178.55)and(135.32±179.44)μg/L,respectively.The levels of IL-9,IL-10,and TGF-β1 in the CHB group were significantly higher than that in the healthy controls[(139.52±193.44)ng/L,(29.73±21.27)ng/L and(5.30±14.51)μg/L,respectively].Different serum levels of IL-9 and IL-10 were detected in patients with different loads of HBV DNA;but there were no correlations between levels of IL-9,IL-10 and TGF-β1 and HBeAg status.IL-9 level showed strong positive correlation with IL-10(r=0.932,P0.001),but weak positive correlation with TGF-β1(r=0.227,P=0.014).IL-10 was slightly correlated with TGF-β1(r=0.217,P=0.018).Conclusions IL-9,IL-10 and TGF-β1 might be involved in the pathogenesis of CHB.IL-9 and IL-10 may play an important role in anti-inflammation,and their increased levels may be related to the chronic infection status of CHB patients.
