摘 要:【目的】观察清养透解法对MRL/lpr小鼠Th1/Th2细胞因子平衡的影响。【方法】选用24只雌性MRL/lpr小鼠随机分为中药组、西药组、中西医结合组、模型对照组,另取6只雌性BALB/c小鼠为正常对照组。中药组给予清养透解合剂(剂量为9.5 g.kg-1.d-1),西药组给予强的松(剂量为10 mg.kg-1.d-1)灌胃,中西医结合组给予强的松合并清养透解合剂灌胃,模型对照组给予生理盐水灌胃,给药6周,正常对照组则不给任何处理。6周后取血检测干扰素(IFN-γ),白细胞介素(IL-12、IL-10、IL-4)水平,并观察小鼠肾脏病理变化。【结果】各治疗组小鼠血清IFN-γ、IL-12水平无显著变化(P>0.05);中药组、西药组、中西医结合组与模型对照组比较,均能显著降低血清IL-10、IL-4水平,提高血清IFN-γ/IL-4比值,差异有统计学意义(P<0.05或P<0.01),中西医结合组能显著升高血清IL-12/IL-10比值(P<0.01);各治疗组均能不同程度改善肾组织病理形态变化。【结论】清养透解法对系统性红斑狼疮的治疗作用与其能调节Th1/Th2细胞因子平衡,改善肾组织炎性病理改变有关。Objective To observe the effect of therapy of clearing heat, nourishing yin and expelling pathogens on the imbalance of Thl and Th2 cytokines in MRL/lpr mice. Methods Twenty-four female MRL/lpr mice were equally randomized into four groups, i. e. , Chinese medicine group, western medicine group, Chinese and western medicine group, and model group. Six female healthy BALB/C mice were in the normal control group. Chinese medicine group was given gastric gavage of herbal mixture (9.5 g ·kg^-1·d^-1 ) with the actions of clearing heat, nourishing yin and expelling pathogens, western medicine group was given prednisone (l0 mg ~ kg1 ~ d'l), Chinese and western medicine group was given the herbal mixture and prednisone, and the model group was givennormal saline orally. The treatment lasted 6 weeks. No medication was given for the normal control group. Six weeks later, serum interferon gamma ( IFN-~/), interleukin 12 ( IL-12), interleukin 10 ( IL-10), and interleukin 4 (IL-4) levels were detected, and the renal pathological changes in mice were also observed. Results Among the medication groups, the differences of IFN-γ/and IL-12 were insignificant (P 〉 0.05 ). Serum IL-10 and IL-4 levels were lower, and the ratio of IFN-γ/to IL-4 was higher in the three medication groups than those in the model group (P 〈 0. 05 or P 〈 0. 01 ). The ratio of IL-12 to IL-10 was the highest in Chinese and western medicine group ( P 〈 0. 01 ). The renal pathological changes were relieved to various degrees in the three medication groups. Conclusion The therapeutic mechanism of therapy of clearing heat, nourishing yin and expelling pathogens for systemic lupus erythematosus is probably related with the regulation of Thl and Th2 cytokine balance and with the relief of renal pathological changes.
