组织样麻风瘤1例

摘　　要：患者男,30岁。躯干、下肢散在结节,眉毛稀疏十个月。皮肤科情况:躯干、四肢散在大小不等的结节,双侧眉毛稀疏,双侧眶上神经和耳大神经明显粗大,踝以下感觉减退。皮损组织病理示:表皮萎缩,可见无浸润带,真皮可见团块状组织样细胞浸润,细胞交织排列成席纹状,神经肿大,周围单一核细胞为主的中度浸润,未见典型的泡沫细胞。抗酸染色可见组织细胞内外大量阳性棒状杆菌。诊断:组织样麻风瘤。A 30-year-old male presented with scattered nodules on lhe trunk and lower limbs, scanty eyebrows for 10 months. Physical examination revealed inequality of size nodules on the trunk, lower limbs, and scanty eye- brows. Neurologic examination demonstrated significant thickness of both supraorbitM nerves and great auricu- lar nerves. Hypoesthesia and anesthesia of both ankles were found. Histopathology of skin lesion showed thin epidermis with Grenz zone, dense histiocytic infiltrate in the dermis forming storifornl patterns, and dermal nen＇es were swollen by cellular infiltration, but the foamy change was not found. Lepta stain showed many acid-fast bacillus. A diagnosis of histoid leprosy was made.
