摘　　要：目的通过检测胃癌癌前病变(PLGC)不同中医证型患者胃黏膜组织CDX2基因的表达,探讨不同中医证型与CDX2基因表达之间的相关性及CDX2表达与不同病理类型之间的关系。方法将24例健康人及120例PLGC患者分为健康对照组、脾虚气滞证组、阴虚有热证组、胃络瘀阻证组,每例取新鲜胃黏膜6块,包括胃体、胃角和胃窦各2块,经免疫组织化学染色观察CDX2基因的表达。结果 CDX2基因阳性表达在脾虚气滞证中为16.67%(6/36),阴虚有热证中为52.17%(24/46),胃络瘀阻证中为63.16%(24/38),健康对照组无表达;其在Ⅰ型肠化生表达为33.33%(18/54),Ⅱ型肠化生表达为25.93%(14/54),Ⅲ型肠化生表达为27.78%(15/54),异型增生表达为12.96%(7/54)。结论 CDX2基因的表达与病理类型有关,在不同中医证型的表达有统计学意义,可为PLGC的中医证型客观化提供依据。Objective To investigate the relation between the different TCM syndromes and CDX2 gene by testing on CDX2 gene expression in gastric mucosa in different TCM syndromes of precancerous lesions of gastric cancer（PLGC）.Methods One hundred and twenty PLGC patients and 24 healthy people were divided into 4 groups,namely,group of spleen deficiency and qi stagnation,group of heat caused by yin deficiency,group of blood stasis in stomach collaterals and healthy control group.Six fresh gastric mucosa tissues were selected（each two tissues of sinuses ventriculi,gastric angle and corpus ventriculi）,and CDX2 gene expression was observed by immunohistochemical staining.Results The positive rates of CDX2 gene expression in different groups were as follows：16.67%（6/36） in group of spleen deficiency and qi stagnation,52.17%（24/46） in group of heat caused by yin deficiency,63.16%（24/38） in group of blood stasis in stomach collaterals,0 in healthy control group;33.33%（18/54）,25.93%（14/54）,27.78%（15/54） and 12.96%（7/54） in intestinal metaplasia Ⅰ,Ⅱ,Ⅲ and dysplasia.Conclusion The positive rates of CDX2 gene expression is related with pathological types,and is different in different syndromes.It lay a foundation for the objectification of TCM syndrome differentiation of PLGC.
