乳腺癌腋窝淋巴结评价的初步探讨

摘　　要：目的探讨自带体部线圈在显示及评价乳腺癌腋窝淋巴结性质的可行性。方法应用GE 1.5T Signa TwinSpeed with Excite超导型MRI扫描仪的自带体部线圈,对20例空芯针穿刺证实为乳腺癌的患者行MRI横断位FSE T2加权成像,记录显像淋巴结的个数及形状,测量其最大径和最小径,并与专用乳腺相控阵表面线圈MRI横断位FSE T2加权成像及病理结果比较。结果①20例乳腺癌患者中,17例病理证实有腋窝淋巴结转移（共35枚淋巴结,包括1例双侧腋窝淋巴结转移患者）。②17例病理证实腋窝淋巴结转移的患者设备自带体部线圈FSE T2WI序列扫描后腋窝均显示肿大淋巴结,共35枚,检出率为100.0%（35/35）;而专用乳腺相控阵表面线圈T2WI扫描仅显示12枚肿大淋巴结（34.3%,12/35）,两者在显示腋窝淋巴结方面有统计学差异（χ~2=34.255,P〈0.01）。③体部线圈T2WI序列扫描显示的35枚淋巴结中,平均最长径为20.6mm（10.6～50.7 mm）,平均最短径为16.4 mm（9.3～28.6 mm）;形状为球形20枚、椭圆形8枚和不规则形7枚。结论 1.5TMRI扫描仪设备自带体部线圈行横断位FSE T2WI序列可有效显示腋窝淋巴结。Objective To study the feasibility of the evaluation of axillary lymph nodes in breast cancer using equipment bringing body coil.Methods Twenty brease cancer patients confirmed by core neddle biopsy underwent axial FSE T2-wighted MRI（T2WI） scanning using the bringing body coil of GE 1.5T Signa Twin Speed with Excite;The number and shape of axillary lymph nodes shown on T2WI were recorded,and the longest and shortest diameter were measured,too.Then,the data were compared with the results of the coil to be used specially for breast and pathology.Results 17 of 20 breast cancer patients had axillary lymph nodes metastasis（totally 35 lymph nodes,1 bilateral axillary lymph nodes metastasis patient included）.35 enlarged axillary lymph nodes in 17 patients confirmed axillary lymph nodes metastasis were shown on FES T2WI achieved by the equipment bringing body coil,with the detection rate of 100%（35/35）.While,only 12 axillary enlarged lymph nodes（34.3%,12/35） were displayed on the coil to be used specially for breast,with significant difference（χ~2 =34.255,P〈0.01） in displaying axillary lymph nodes.Among the 35 lymph nodes displayed on T2WI obtained from body coil,the mean longest and shortest diameter were 20.6 mm（10,6-50.7 mm） and 16.4 mm（9.3-28.6 mm）,respectively.And the numbers of round,oval and irregular were 20,8 and 7, respectively.Conclusion Axial FSE T2WI scanning using the bringing body coil of GE 1.5T Signa Twin Speed with Excite could effectively display axillary lymph nodes.
