乳腺微钙化术前导丝定位效果评价及钙化形态分析

摘　　要：目的评价术前导丝定位对于乳腺微钙化切除的指导效果及微钙化分析。方法乳腺X线摄影发现87例微钙化,BI-RADS分级为4a～5级。利用GE公司乳腺摄影机二维立体定位系统,患者术前导丝定位,术后标本钼靶摄片并送病理检验。结果 78例导丝定位成功,9例定位于微钙化周围;87例一次切除成功。在87例微钙化中发现31例恶性病变,其中3例导管原位癌、28例浸润性导管癌;纤维腺瘤13例;导管内乳头状瘤14例;乳腺病29例。其中检出恶性病变4a级比例为9.4%,4b级比例为29.6%,4c级比例为57.9%,5级比例为100%。结论乳腺微钙化术前导丝定位成功率高。根据BI-RADS分级进行乳腺微钙化导丝定位穿刺,对于乳腺微钙化的诊断和切除具有良好的指导效果。Objective To assess the effect of fine needle localizations and the mammographic appearance of a cluster of microcalcifications.Methods 87 women with microcalcifications underwent fine needle localizations before resection,and then the specimens was analyzed by mammography and pathiology.Results 78 cases showed that the wire and microcalcification were located in surgical specimens.9 cases were located around the microcalcifications.The success rate of resetion for the first time was 96.6%. 31 cases had malignant lesions in final pathologic diagnosis.3 cases were carcinomas in situ,28 cases were invasive ductal carcinomas. 13 cases were fibroadenomas.14 cases were intraductal papillomas.29 cases were breast adenosis.The detective proportion of BI-RADS 4a,4b,4c and 5 was 9.4%,29.6%,57.9%and 100%.Conclusion Guide-wire positioning system has a good effect on resection guide and diagnosis of breast microcalcification according to the grades of BI-RADS.
