透明质酸钠凝胶在重度宫腔粘连松解术中的应用及疗效分析

摘　　要：目的探讨医用透明质酸钠凝胶用于重度宫腔粘连(IUA)分离术后预防再粘连的临床疗效。方法将2011年1月—2012年1月来我院就诊、经宫腔镜确诊的重度IUA患者48例随机分为观察组和对照组,对照组在重度IUA分离术后使用宫内节育器(IUD),观察组采用IUD+医用透明质酸钠凝胶。通过二探法观察两组患者的IUA面积、粘连物血管生长程度、粘连物韧性及术后月经、妊娠情况等。结果两组患者宫腔再粘连率、再粘连程度及月经改善率经比较,差异均有统计学意义(P<0.05);两组妊娠率经比较差异无统计学意义,考虑与观察时间短有关。结论重度IUA分离术后于宫腔内放置医用透明质酸钠凝胶可有效预防再粘连,提高月经改善率及妊娠率。OBJECTIVE To investigate medical sodium hyaluronate gel for severe intrauterine adhesions after commissurotomy preventing readhesion of clinical curative effect. METHODS To prospectively observed severe intrauterine adhesions dissection after using the IUD（control group）or IUD ＋ medical sodium hyaluronate gel（observation group）, by two probe method observation of intrauterine adhesion area, adhesion complexes of vascular growth degree of toughness, adhesion complexes and after menstruation, pregnancy and other. RESULTS IUD and IUD ＋ medical sodium hyaluronate group palace cavity adhesion rate, adhesion, menstrual improvement rates, the differences were statistically significant（P〈0.05）, two groups of pregnancy rate is not statistically significant differences, consider short observation time on. CONCLUSIONS Severe intrauterine adhesions dissection of intrauterine cavity of medical sodium hyaluronate gel is effective in the prevention and adhesion and improve menstrual improvement rate.
