摘 要:目的 探讨合理的螺旋CT门静脉造影层厚和重建方法。材料与方法 将 16 0例疑有肝脏疾病进行螺旋CT门静脉造影的患者随机分为两组 ,3mm层厚组 98例 ,5mm层厚组 6 2例。造影剂剂量为 2ml/kg(体重 ) ,注射速率 3ml/s ,延迟时间 45~ 5 0s。每一患者均以MIP和MPVR行门静脉成像 ,并对其图像进行 5个单项和综合评分。结果 3mm层厚组和 5mm层厚组MIP图像平均综合评分分别为 10 .8和 8.9,而MPVR图像分别为 12 .5和 10 .7,相同重建方法时两组之间差异均有统计学意义 ,同组内不同重建方法之间差异也均有统计学意义 ,3mm层厚组与MPVR在显示门静脉、肝静脉级别和血管边缘清晰度方面优于 5mm层厚组与MIP。结论螺旋CT门静脉造影层厚以 3mm优于 5mm 。Objective To optimize the collimation and the reconstructive method in spiral CT portography (CTP).Materials and Methods CTP was performed in 160 cases suspected with liver disease, who were randomly divided into 2 groups and scanned with 3mm collimation (n=98) and 5mm (n=62), respectively. The contrast dose was 2ml/kg weight with the injection rate of 3ml/s. The delay time was within 45~50s. Both MIP and MPVR were used to reconstruct the portal veins in all patients. The images were evaluated and scored based on the grade of portal vein, CT value difference between the portal vein and the liver parenchyma, the definition of blood vessel edge, the grade of hepatic vein and the display of IVC by two radiologists.Results The average combined score of MIP images in 3mm and 5mm groups was 10.8 and 8.9, while MPVR was 12.5 and 10.7, respectively. For visualizing the portal vein and hepatic vein and the clearness of blood vessel margin, 3mm collimation group and MPVR were superior to 5mm group and MIP.Conclusion In spiral CT portography, it is of optimal choice to use 3mm collimation and MPVR reconstructive method.
