伴抑郁焦虑状态的功能性消化不良中医证型研究

摘　　要：目的:探讨伴抑郁焦虑状态的功能消化不良中医证型的状况。方法:选取某医院在2010年1月～12月这一年中诊断为功能性消化不良(Functional Dyspepsia,FD)的患者并且伴有抑郁焦虑状态的数量一共150例,对这些患者进行中医证型的调查,对各类证型分布和功能性消化不良(FD)症状谱进行比较分析,连同焦虑自评量表(Self-rating Anxiety Scale,SAS)和抑郁自评量表(Self-rating Depression Scale,SDS)的评分差别。结果:脾虚气滞证是中医证型分布中最为常见的现象;功能性消化不良(FD)症状和中医证型相比,其脾虚气滞代表餐后饱胀感和上腹痛,脾胃气虚代表早饱,湿热滞胃代表上腹烧灼感;SAS量表、SDS量表和中医各证型的评分相比较,SAS、SDS量表的分数最高是在患者脾虚气滞证时,两个表分数最低时是在湿热滞胃证时。结论:伴抑郁焦虑状态的功能性消化不良中医各证型的分布状况和症状明显,最为常见的为脾虚气滞,本虚以脾虚为主,标实以肝气郁结、湿阻气滞和胃气失降为主,病位以肝、胃和脾为主。Objectivei To investigate the TCM syndrome of FD with depression and anxiety status. Methods： Select 150 cases of patients FD with depression and anxiety status in one hospital from January 2010 to December, the syraptoms of various types of card distribution and FD spectra were compared, at the same time, to compare the SAS and SDS score difference. Results： Spleen Qi stagnation is the most common phenomenon of TCM syndromes; to compare the TCM syndrome, the Pixu Qizhi of FD symptoms represented the sense of postprandial fullness and abdominal pain, spleen and stomach Qixu represented as early as full, Shire Zhiwe~ on behalf of epigastric burning sensation; the scale of the SAS and SDS scale and TCM syndrome type of score compared with with each other, the highest scores on the SAS and SDS in patients with Pixu Qizhi, the lowest score of the two tables in patienlLs with Shire Zhiwei. Conclusion The distribution and symptoms of TCM syndrome with depression and anxiety state is obvious, Pixu Qizhi is the most common. Pixu as the main of Bnexu, Ganqi Yujie, ShizuQizhi and Weiqi Shijiang as the main of Biaoshi, Spleen and the virtual to the Spleen, standard real to liver qi stagnation, dampness and qi stagnation and Stomach lost down the main disease location mainly to the liver, stomach and spleen, disease location mainly to the liver, stomach and spleen.
