摘  要:目的为了能最大程度获得高质量和足够数量的精子,根据《WHO人类精液检验与处理实验室手册》第五版要求对体外精液优选技术进行了改进,以期为患者提供一种更安全、可靠、高效的精液优选方法。方法收集2011年1至9月广东省中医院生殖医学科共计68对不育夫妇的98份精液标本,采用改良前后的精液优选方法(密度梯度法和上游法)分别进行精液优选处理。优选前后使用改良Neubauer血细胞计数器进行精子浓度分析,西班牙SCA精子分析仪测定精子活动力,改良巴氏染色进行精子形态学分析。结果改良后两种方法回收的精液总活动精子数、前向运动(a+b)精子总数、回收率有所提高(P〈0.05);其精子浓度、正常形态精子百分率未有明显变化(P〉0.05)。优选前后比较正常形态精子有显著提高(P〈0.01)。结论改良后精液优选技术能提高患者的前向运动精子总数及回收率,有效提高不育夫妇受孕的机率。Objective To improve in vitro sperm selection technology basing on WHO lab mamual for Examination and Process of Human Semen ( fifth edition ) to offer patients with a safer, more reliable and effective sperm selection procedure. Methods 98 specimens from 68 sterility couples were collected. The specimen were detected by the old selection technology and the modified technology ( Direct swim-up and Discontinuous density-gradients ). Neubauer Blood Cell counter was used to detect the sperm concentration. Spain SCA-CASA was used to assess sperm motility and Papanicolaou Staining was used to detect sperm morphology, ltesults The forward sperm quantity and recovery im- proved after modification ( P〈 0.05 ). The sperm concentration and normal forms of sperm did not change after modification ( P 〉 0.05 ). The count of sperm with normal forms increased after selection ( P 〈 0.01 ). Conclusions Modified sperm selection technology can improve the quantity of forward sperm and the recovery rate. It can increase the chance of conception in sterility couples.
