斑点追踪显像对重度主动脉瓣狭窄左心功能的评价

摘　　要：目的:探讨斑点追踪显像(STI)评价射血分数(EF)正常的重度主动脉瓣狭窄(AS)患者左心室收缩和舒张功能的价值。方法:45例重度AS患者,42例正常人,测量长轴三节段纵向应变率(SrL)和短轴三水平径向应变率(SrR)、圆周应变率(SrC)及旋转率(RotR)的收缩期S峰值、舒张早期E’峰值、舒张晚期A’峰值及E’/A’值。结果:与正常组相比,重度AS组SrL、SrR和SrC的S、E’、E’/A’均降低,SrI、SrR和SrC的A’及RotR的S、E’升高。结论:STI检测重度AS左心室功能的受损不仅为舒张功能,而且收缩功能亦同时受累。左室EF正常的重度AS患者存在收缩功能异常。Objective：To observe the value of speckle tracking imaging （STI）in assessing changes of left ventricular diastolic and systolic func- tion in severe aortic stenosis （AS）patients with preserved left ventricular （LV） ejection fraction （EF）.Methods：Forty-five AS patients,and forty-two normal persons out of them were recruited as study subjects.Peak systole（S）,peak early diastole（E＇ ）,peak late diastole（A＇ ）and E＇ /A＇ratio at longitudinal strain rate （SrL）of three segments in long-axis views and radial strain rate （SrR）, circumference strain rate（SrC ）and rotation rate（ROtR）of three levels in short-axis views were measured.Results ： Compared with controls,severe AS group had significantly de- creased S.E＇and E＇/A＇at SrL.SrR and SrC,increased A＇at SrL.SrR and SrC andS. E＇at RotR.Conclusions：It was indicated bv STI that LV dysfunction in severe AS was appeared on diastolic as well as systolic dysfunction. Left ventricular ejection fraction normal severe AS patients with narrow exist systolic function abnormalities.
