骨科手术肢体缺血再灌注后肌钙蛋白I的动态变化及临床意义

摘　　要：目的:观察骨科手术肢体缺血再灌注后肌钙蛋白I的动态变化,为围术期心肌保护提供临床依据。方法:选择20例下肢骨科手术病人,均于单侧下肢使用止血带,压力80kPa,持续时间不超过90min。分别在扎止血带前(T0),松止血带后10min(T1),30min(T2),1h(T3),3h(T4),6h(T5)等6个时点,采集静脉血2ml,分离血清,检测肌钙蛋白I(cTnI)及心肌酶谱。结果:在各时间段,cTnI的变化具有显著性差异(P<0.01)。cTnI呈现先高后低的特点,在松止血带后10min后开始升高,松止血带后60min,cTnI达到最高值,然后逐渐下降。结论:cTnI的动态变化特点为临床采用适当的干预方法提供了依据。Objective： To observe cinical change of Troponin I in patients poceeding orthopedics Operation using tourniquet. Methods： 20 pa- tients with lower extremity orthopedic surgery were chosed, and lower limb tourniquet pressure 80 kPa, duration time 〈90 rain. Troponin I （cTnI） was detected before relaxing tourniquet （T0）, 10min （T1） ,30min（T2）, lh （T3）, 3h （T4）and 6h（Ts） after relaxing tourniquet. Results： Compared with that at To, cTnI at T1, T2,T3,T4 and T5 increased markedly （P〈0.01）, and cTnI at T3 was the highest value. Conclusion： Rea- sonable clinial interference methods was necessary according to change of Troponin I.
