输尿管硬镜出镜困难的防治(附16例报告)

摘　　要：目的:探讨输尿管硬镜手术时出镜困难的防治措施。方法:回顾分析输尿管硬镜手术发现较明显出镜困难的16例病例,10例为取石钳嵌顿、6例为镜体嵌顿。结果:10例取石钳嵌顿病例通过松开取石钳、回推镜体后8例顺利出镜,2例改行开放手术;6例镜体嵌顿病例旋转镜体后3例顺利出镜,3例改行开放手术。结论:克服输尿管出镜困难的要点在于及时预防、熟悉输尿管解剖与操作技巧。Objective ：To study and analysis the reasons of difficult accesses by manipulating rigid transurethral ureteroscopy and to find out the corresponding solving methods.Methods： 16 cases occurring the difficuties in the ureteroscopy exsertion were evaluated retrospectively from March 2007 to December 2011.Results： 11 cases can be success to achieve by rotating the ureteroseopy, push back the ureteroscopy and loosen stone forceps. 5 cases were converted to open surgery.Conclusion： Difficulty in the ureteroscopy exsertion is not rare,we should be familiar with the anatomy of ureter and master the techniques of operation.
