摘　　要：目的观察骶骨标本第一骶孔的解剖学特点，为第一骶前孔神经阻滞提供解剖学依据。方法对50例骶骨标本进行测量，包括左、右第一骶后孔与骶正中嵴的距离，第一骶前后孔的深距，并确定穿刺进针的最佳角度。结果左、右第一骶后孔与骶正中嵴的距离分别为（23．6±1．4）mm和（23．5±1．8）mm。左、右侧第一骶前、后孔深距分别为（26．1±2．8）mm和（25．4±2．6）mm。最佳进针角度，左侧：与冠状面和矢状面夹角分别为（82．3±2．6）°和（8．3±1．1）°；右侧：与冠状面和矢状面夹角分别为（83．9±2．9）°和（9．2±1．5）°。结论第一骶前孔神经阻滞穿刺点宜选择距骶正中嵴旁开25～30mm，角度以与冠状面82°～84°、矢状面8°-10°较易穿刺成功。Objective To observe the anatomical characteristics of the first sacral foramina of sacrum specimens so as to provide anatomic basis for the first sacral foramina nerve block. Methods 50 sacrum specimens were measured, including the distance between the median sacral crest and the left and right first posterior sacral foramina, the distance between the first anterior and posterior sacral foramina. The best angle for needle insertion during the first anterior sacral foramina nerve block was determined. Results The distance between the median sacral crest and the left and right first sacral foramina was （23.63 ± 1.41 ） mm and （23.50 ± 1.77 ） mm respectively. The distance between the first anterior and posterior sacral foramina was （26.1± 2.8 ） mm and （ 25, 4 ± 2.6） mm respectively. The best angle for needle inser- tion, left sacral foramina ： the angle to the coronal plane and sagittal plane was （ 82.3 ± 2.6）° and （ 8.3 ± 1.1 ） ° respec- tively ; right sacral foramina ： the angle to the coronal plane and sagittal plane was （83.9 ± 2.9 ） ° and （9.2 ± 1.5 ）° re- spectively. Conclusion For the first sacral foramina nerve block,it is more easy to insert the needle successfully when the insertion point is at 25 -30 mm from the median sacral crest and at an angle of 82° -84°to the coronal plane and 8° - 10°to the sagittal plane.
