摘 要:目的比较经阴道二维超声(2D-HyCoSy)和三维子宫输卵管超声造影(3D-HyCoSy)评价输卵管通畅性的准确性及其临床应用价值。方法对47例不孕症妇女分别采用2D-HyCoSy和3D-HyCoSy评估输卵管通畅性,其中14例行腹腔镜下通染液检查。对比2D-HyCoSy和3D-HyCoSy检测输卵管通畅性的敏感性、特异性、阴性预测值及阳性预测值。结果 2D-HyCoSy评估输卵管通畅性的敏感性、特异性、阴性预测值及阳性预测值分别为69.2%、100%、90.0%及87.5%;3D-HyCoSy的敏感性、特异性、阴性预测值及阳性预测值分别为84.6%、100%、91.6%及100%,两者比较差异无统计学意义。2D-HyCoSy和3D-HyCoSy分别与腹腔镜下通染液检查结果比较,吻合度均较高(K=0.730,K=0.837),差异均有统计学意义(P﹤0.001)。结论 2D-HyCoSy和3D-HyCoSy均可准确、快速地评价输卵管通畅性,3D-HyCoSy对输卵管全程立体空间走行的显示更具优势。Objective To explore the accuracy and clinical application value of two-dimensional hysterosalpingo-contrast sonography ( 2D -HyCoSy) and three-dimensional hysterosalpingo-contrast sonography ( 3 D-HyCoSy) in evaluation of tubal patency. Methods Tubal patency in 47 cases of infertile women was evaluated using 2D-HyCoSy and 3D-HyCoSy. Among them, 14 patients accepted the examination of laparoscopy. The sensitivity, specificity, negative predictive value and positive predictive val- ue of evaluating tubal patency were compared. Results The sensitivity, specificity, negative predictive value and positive pre- dictive value of 2D-HyCoSy were 69.2%, 100%, 90.0% and 87.5%, respectively. The sensitivity, specificity, negative predictive value and positive predictive value of 3D-HyCoSy were 84.6%, 100%, 91.6% and 100%, respectively. There were no significant differences.While there were significant differences compared 2D-HyCoSy and 3I)-HyCoSy with laparoscopy (P 〈 0.001 ), good- ness of fit were higher (K=0.730, K=0.837). Conclusion Both 2D-HyCoSy and 3D-HyCoSy can diagnose tubal patency accu- rately and quickly, while 3D-HyCoSy has more advantages in image display.
