超声评价长期饮酒者肱动脉内皮功能与左室舒张功能的关系

摘　　要：目的超声评价长期饮酒者肱动脉内皮功能与左室舒张功能,并探讨其相关性。方法 80例男性饮酒者根据饮酒量和饮酒时间分为A组30例,B组25例和C组25例;选取30例健康男性志愿者为正常对照组。超声检测各组饮酒者及正常对照者的血管内皮依赖性舒张功能(EDD)。应用定量组织速度成像(QTVI)技术检测二尖瓣环的舒张早期峰值速度(Ve)、舒张晚期峰值速度(Va)及两者之间的比值(Ve/Va)。结果与正常对照组和A组比较,B、C组的EDD显著减小,Va明显增大,Ve、Ve/Va显著减低,差异有统计学意义(P﹤0.01);与B组比较,C组的EDD显著减小,Ve减低,差异有统计学意义(P﹤0.05),Ve/Va及Va变化不明显。与正常对照组比较,A组各参数差异均无统计学意义。相关分析显示EDD与Ve和Ve/Va均呈正相关(r=0.720、0.524,P=0.000)。结论长期大量饮酒可导致血管内皮功能障碍及左室舒张功能减退,血管内皮功能障碍与左室舒张功能不全密切相关。Objective To evaluate the relationship between brachial artery endothelial function and left ventricular diastolic function in the long-term drinkers by uhrasonography.Methods Eighty male drinkers were divided into 3 groups according to alcohol intake and drinking time, including group A （n=30）, group B （n=25） and group C （n=25）.Thirty healthy male volunteers were served as normal control group （n=30）.The endothelium-dependent dilation function （EDD） of the brachial artery was measured by high-frequency ultrasonography.Ve, Va and Ve/Va were detected by quantitative tissue velocity imaging（QVTI）. Results Compared with the control group and A group, EDD reduced, Va increased,Ve and Ve/Va reduced significantly in groups B and C （P 〈 0.01 ）.Compared with B group, EDD and Ve reduced in group C （P 〈 0.05 ）.There was no significant difference of Va and Ve/Va between two groups.There was no significant difference of all parameters between control group and A group. Correlation analysis showed that the EDD was positively correlated with Ve and Ve/Va （r=0.720,0.524,P=0.000）.Conclusion The disorders of the brachial artery endothelial function may related closely with left ventricular diastolic disfunetion in the long- term drinkers.
