摘  要:目的比较ELISA、GICA与MEIA法对新生儿TORCH-IgM抗体的检测效果。方法选择120例疑似TORCH感染的新生儿作为研究对象,分别采用ELISA、GICA与MEIA三种方法检测血清TORCH-IgM抗体,比较三种方法对新生儿TORCH-IgM抗体检测的阳性率、符合率、灵敏度以及特异性。结果以MEIA法为金标准,ELISA法和GICA法检测TORCH-IgM抗体的灵敏度和特异性比较,ELISA法检测中的RV-IgM、HSV-IgM以及CMV-IgM检测的灵敏度均高于GICA法(P<0.01),两组的TOX-IgM检测比较无显著性差异(P>0.05);ELISA法检测中的CMV-IgM检测的特异性高于GICA法(P<0.01);ELISA法和GICA法检测TORCH-IgM抗体阳性预测值和阴性预测值比较,ELISA法中CMV-IgM检测的阳性预测值和阴性预测值均高于GICA法(P<0.05),尤其是阴性预测值(P<0.01);与MEIA法检测总符合率均在90%以上。结论 ELISA法检测新生儿TORCH-IgM抗体,具有特异性强、敏感性高、可信度高、简单易行等优点,是一种比较可靠的新生儿TORCH感染筛查的检测方法。Objective To compare the detection effect of ELISA,GICA and MEIA on TORCH-IgM antibody in newborn.Methods 120 newborn with suspected TORCH infection were chose as study object.Their serum TORCH-IgM antibody were detected with ELISA,GICA and MEIA,respectively.The positive rate,coincidence rate,sensitivity and specificity of three methods were comparedResults To the gold standard for MEIA method,RV-IgM,HSV-IgM CMV and the sensitivity of testing-IgM of ELISA were higher than GICA method(P0.01) when ELISA and GICA method to detect the TORCH-IgM antibody specificity and sensitivity comparison,and there was no significant difference on TOX-IgM(P0.05).Take MEIA as golden standard,the CMV-IgM coincidence rate of ELISA is higher than that of GICA(P0.05) when ELISA and GICA method to detect TORCH-IgM antibody positive and negative prediction is predicted.Sensitivity detection of CMV-IgM,HSV-IgM and CMV-IgM in ELISA are higher than that in GICA(P0.01),and CMV-IgM specificity detection of ELISA is higher than that of GICA(P0.01).The positive predictive value and negative predictive value of CMV-IgM detection in ELISA are higher than that in GICA(P0.05),especially for negative predictive value(P0.01).And MEIA method to detect the total coincidence rate were above 90%.Conclusion There are high sensitivity and specificity of strong,high reliability,simple,etc,when ELISA test neonatal TORCH-IgM antibody.It is a kind of reliable newborn screening TORCH infection test methods.
