216例妊娠期高血压患者回顾性调查分析

摘　　要：目的对不同类型的妊娠期高血压疾病(hypertensive disorder complicating pregnancy,HDCP)患者的发病情况、治疗方法及母婴结局进行分析和探讨。方法对2009年6月-2011年3月收治的216例HDCP患者进行回顾性分析。结果 216例患者中,重度子痫前期病例最多,占57.41%;不同病情严重程度组别间年龄、孕妇居住地(城市或农村)、有/无定期产前检查存在相关性;随着病情严重程度不同,期待治疗时间比较差异有统计学意义;重度子痫前期和子痫组比轻度子痫前期组的早产儿、死胎与胎儿宫内发育迟缓(FGR)的发生率高,差异有统计学意义(P<0.01)。结论妊娠期高血压疾病潜在危害大,早认识、早发现和早就诊是预防关键。严格选择病例,期待治疗过程中严密监测母体胎儿病情变化适时手术,是保证母婴安全的有效手段。Objective To analyze and explore the incidence,treatment and outcomes of patients with different types of hypertensive disorder complicating pregnancy（HDCP）.Methods A retrospective study was carried out on 216 HDCP patients who were hospitalized in the Maternal and Child Health Hospital of Hengyang City from June 2009 to March 2011.Results Among 216 patients,most of them were diagnosed with severe pre-eclampsia,accounting for 57.41%.There was a correlation in age,living area of pregnant women（city or countryside）,and regular antenatal care among patients with various degrees of HDCP.There were statistically significant differences in expectant treatment time among patients with various degrees of HDCP.The incidence rates of premature infant,fetal death,and intrauterine growth retardation in patients with severe pre-eclampsia and eclampsia were higher than those in patients with light pre-eclampsia,and the difference was statisticallysignificant（P0.01）.Conclusions There are potential hazards of being suffered HDCP,early recognition,early detection and early treatment are the key points to prevent HDCP.Strictly choosing the cases,closely monitoring the changes of fetal-maternal conditions and timely surgical therapy during the period of expectant treatment are the effective methods to ensure the maternal and fetal safety.
