探讨232例院际转运先天性心脏病危重患儿的安全性及其转归

摘　　要：目的探讨先天性心脏病危重症患儿院际转运的安全性。方法回顾性分析2009年8月至2011年7月院际转运的232例先天性心脏病危重症患儿的转运前、中和到儿科监护病房(pediatric intensive care unit,PICU)后的临床资料、转运距离、时间和患儿的转归。结果232例患儿,男152例,女80例;年龄1个月～12岁,中位年龄5个月;体质量2.5～25 kg,中位体质量5.5 kg;转运花费时间20～990 min,中位时间156 min;转运距离是74～620 km,中位距离204 km,其中47.2%的患儿转运距离100～200 km,中位距离148 km。转运过程中需要呼吸机正压通气的患儿56例(24.1%),无1例死亡。转运的患儿均入住广东省人民医院PICU,其中治愈及好转220例(94.8%),放弃治疗9例,死亡3例。结论配备专业的转运队伍,做好转运前的充分准备,转运中的密切监护和及时的处理,能提先天性心脏病高危重症患儿的院际转运安全性和预后。Objectives To discuss the security of interhospital transport of critically ill children with congenital heart disease （CHD）. Methods The clinical data, transport distance, duration and outcomes of 232 critically ill children with CHD who were transported from other hospitals in a period from August 2009 to July 2011 were analyzed retrospectively. Results Among 232 critically ill children with CI-ID, there were 152 males and 80 females;their ages ranged from 1 month to 12 years old （median 5 months） ; weights ranged from 2.5 kg to 25 kg （median 5.5 kg） ; transport durations ranged from 20 min to 990 min （median 156 min） ; transport distances ranged from 74 km to 620 km （median 204 km）, and 47.2% of them were transported from hospitals 100 km to 200 km away （median 148 km）. None of the patients died and all of them were hospitalized in the pediatric intensive care unit （PICU） of Guangdong General Hospital. There were 56 （24.1% ） children required ventilation during transport. In the end, 220 patients were cured or improved, 9 of them gave up treatment, 3 of them died. Conclusions A specialized retrieval team with adequate preparation before transport, close observation and monitoring on the way can effectively lower mortality and disability rates of critically ill children with CHD and improve the security and outcomes of interhospital transport.
