摘  要:目的评价超声刀对腹腔镜卵巢囊肿剥除术后卵巢功能的影响。方法选取因双侧卵巢良性囊肿行腹腔镜下卵巢囊肿剥除术的患者80例,根据处理残留卵巢的不同方法随机分为超声刀组和镜下缝合组,每组40例。比较各组手术前后的雌二醇(E2)、卵泡刺激素(FSH)、黄体生成素(LH)以及基础窦卵泡计数(AFC)的变化情况,术后随访12个月。结果全部患者均顺利完成手术,术中未发生任何并发症。两组术后均出现血清E2和AFC降低,FSH和LH增高,组间比较差异无显著性(P>0.05),两组与术前比较差异均无显著性(P>0.05)。结论腹腔镜下卵巢囊肿剥除术中采用超声刀处理残留卵巢并不加重术后卵巢功能的损伤,相比采用镜下缝合法可以减少手术时间。[ Objective ] To investigate the influence on ovarian function after laparoscopic ovarian surgery using ultrasonic scalpel. [Methods ] 80 patients with both ovarian benign cysts who underwent laparoseopic ovarian cys- tectomy were randomly divided into two groups according to the methods of managing residual cvarian parenchyma including ultrasonic sealpe group and suture as the controlled group. Every group has 40 cases. The levels of serum estradiol (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH), antral follicle count (AFC) were detected respectively before the operations and at 6 month after the operations. All patients were followed up for 12 months. [ Results ] All the surgeries were accomplished successfully under laparoseopy without any complication. The opera- fion time in ultrasonic scalpe group was shorter than that in controlled group (P 〈0.05). Comparing with preoperation, the postoperative levels of E2 and AFC were decreased, postoperative FSH and LH were increased in all patients, there were no significant differences between the two groups (P 〉0.05). There were no significant differences for post- operative level of E2, FSH, LH, AFC in two groups (P 〉0.05). [ Conclusions ] Using the ultrasonic scalpel to stanch when stripped of the overian cyst by laparoseope could effectively decrease the operation time, and do not aggravate the damage of the recent ovarian function.
