右美托咪定在尿道前列腺电切术中预防寒战的临床观察

摘　　要：目的:观察右美托咪定经尿道前列腺电切术中寒战的预防作用。方法:择期腰硬联合麻醉下经尿道前列腺电切术者80例,随机分为治疗组(40例。麻醉后予右美托咪定1μg/kg静注,10min完毕,之后以0.5μg.kg-1.h-1静脉泵注维持)和对照组(40例。等量NS静注)。观察两组生命体征变化,Ramsay镇静评分以及寒战发生率和严重程度。结果:治疗组注药5～10min后HR和RR下降(P<0.05),平均动脉压(MAP)组间差异无统计学意义;Ramsay镇静评分较对照组高;寒战发生率治疗组为15.0%,较对照组(37.5%)低(P<0.05)。结论:右美托咪定静注对前列腺电切术患者有良好的镇静作用,能降低术中寒战发生率和严重程度。Objective： To investigate the effect of dexmedetomidine on shivering in patients undergoing transurethral resection of the prostate（TURP）. Methods： Eighty patients undergoing TURP received eanbined-spinal epidural anes- thesia were randomly divided into treatment group（40 cases, given dexmedetomidine i.v. at the dose of 1.0 ttg/kg within 10 minutes and then 0.5 μg.kg-l.h-1）and control group（40 cases, injected saline solution with equal volume）. The circulation and respiration, the sedative grade, the occurrence rate and degree of shiver were observed. Results： The heart rate and respire rate in the treatment group were decreased after dexmedetomidine given （P 〈 0.05）, the mean artery pressure was no different between two groups. Ramsay scores in the treatment group was higher than that in the control group. The occurrence rate of shivering in the treatment group was 15.5%, 15.0% in the control group（ P 〈 0.05）. Conclusion： Dexmedetomidine can show a good effect on shivering in patients undergoing TURP.
