摘  要:目的探讨2型糖尿病(T2DM)患者甲状腺功能、甲状腺球蛋白抗体(TGAb)、甲状腺过氧化物酶抗体(TPOAb)检测的临床意义。方法 210例住院T2DM患者分成甲状腺功能正常组(A组,186例)、甲亢组(B组,8例)和甲减组(C组,16例),检测三组患者游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)及TGAb、TPOAb的水平。结果与A组比较,B组TSH明显减低(P<0.01);C组FT4明显减低(P<0.01),TSH明显增高(P<0.01)。B组及C组TPOAb阳性率分别为25%和50%,均明显高于A组的8.6%(P<0.01)。结论临床上可将TSH和TPOAb作为T2DM患者甲状腺功能的初筛指标。Objective To investigate the clinical significance of detecting the changes of thyroid function, thyroglobulin antibody(TGAb) and thyroid peroxidase antibody(TPOAb) in the patients with type 2 diabetese mellitus(T2DM). Methods A total of 210 T2DM patients was divided into three groups of A(186 cases, with normal thyroid function), B(8 cases, with hyperthyroidism) and C ( 16 cases, with hypothyroidism). Plasma levels of free triiodothyronine ( FT3 ), free thyroxine (FT4), thyroid stimulating hormone(TSH) ,TGAb and TPOAb were detected. Results Compared with group A, plasma TSH titer was significantly lower in group B(P〈0. 01), while plasma FT4 was significantly lower, and TSH titer was significantly higher in group C(P〈0. 01 ). The positive rates of TPOAb in groups of B and C were 25% and 50%, which were significantly higher than 8. 6% in group A (P〈0. 01). Conclusion Plasma TSH and TPOAb could be taken as the indicators for screening the thyroid function in T2DM patients.
