原发性高血压患者颈动脉内膜中层厚度与基质金属蛋白酶的相关性

摘　　要：目的探讨原发性高血压(EH)患者血清基质金属蛋白酶2(MMP-2)、MMP-9及其组织抑制因子1(TIMP-1)的浓度变化和颈动脉内膜中层厚度(IMT)的相关性。方法选取EH患者70例分为A组(40例,IMT≥0.9mm)和B组(30例,IMT<0.9mm);健康体检者为正常对照(C组,36例)。采用超声多普勒测量颈动脉IMT并计算斑块积分;ELISA法检测血清MMP-2、MMP-9、TIMP-1浓度并计算MMP-2、MMP-9与TIMP-1比值。结果 A组血清MMP-2、MMP-9、TIMP-1浓度均显著高于B、C组(P<0.01),而MMP-2/TIMP-1、MMP-9/TIMP-1比值却均显著低于B、C组(P<0.01);A、B组血清MMP-2、MMP-9、TIMP-1浓度与IMT及斑块积分呈显著正相关(P<0.01),而MMP-2/TIMP-1、MMP-9/TIMP-1比值与IMT及斑块积分却呈显著负相关(P<0.05)。结论 EH患者血清MMP-2、MMP-9及TIMP-1与颈动脉粥样硬化的发生和发展有关。Objective To investigate the correlation between carotid IMT and serum matrix metalloproteinase-2 （ MMP-2 ）, matrix metalloproteinases-9 （ MMP-9 ） and tissue inhibitor of metalloproteinase-1（TIMP-1） in patients with essential hypertension（EH）. Methods Serum MMP-2, MMP-9,TIMP-170 were detected with ELISA in 40 EH patients with IMT≥0. 9 mm（group A）, 30 EH cases with IMT〈0.9 mm（group B） and 36 healthy people（group C）. The ratioes of MMP-2 to TIMP-1 and MMP-9 to TIMP-1 were calculated. Results The concentrations of serum MMP-2, MMP-9 and TIMP-1 in group A were significantly higher than those in groups of B and C（P〈0. 01）, while the ratioes of MMP-2 to TIMP-1 and MMP-9 to TIMP-1 in groups of A were significantly lower than those in groups B and C（P〈0. 01）. Serum levels of MMP-2,MMP-9 and TIMP-1 in groups of A and B positively correlated with carotid IMT and plaque score（P〈0. 01） ,while the ratioes of MMP-2 to TIMP-1 and MMP-9 to TIMP-1 were negatively correlated with carotid IMT and plaque score （P〈0. 05）. Conclusion MMP-2, MMP-9 and TIMP-1 are associated with the occurrence and development of carotid atherosclerosis in the patients with EH.
