生殖道感染对未足月胎膜早破妊娠结局的影响

摘　　要：目的:探讨生殖道感染对未足月胎膜早破孕妇的妊娠结局的影响。方法:回顾性分析2011年6月至2011年12月广东省深圳市妇幼保健院收治的126例未足月(孕周为28～37周)胎膜早破孕妇的临床资料。根据检验结果将患者分为非感染组(33例),细菌感染组(30例),非细菌感染组(32例)和混合感染组(31例)。比较4组患者的入院孕周、分娩孕周、保胎天数、新生儿出生体质量、宫内感染率、胎盘病理阳性程度、分娩方式、新生儿窒息程度、新生儿并发症、产妇并发症发生率等指标。结果:细菌感染组与混合感染组患者的分娩孕周、保胎时间明显缩短,新生儿体质量明显降低,胎盘病理阳性率及宫内感染率明显增加(P<0.05)结论:加强对未足月胎膜早破孕妇的生殖道细菌的培养及检测,及时、有针对性的治疗,可降低早产发生率。Objective：To investigate the impact of reproductive tract infection （RTI） on pregnancy outcome in preterm premature rupture of membrane （PROM）. Methods：Between June 2011 and December 2011,126 pregnant women （gestational weeks： 28-37 weeks） with preterm PROM registered in Shenzhen Women＇s and Children＇s Hospital in Guangdong were retrospectively analyzed and divided into non-infection group （n = 33 ）, bacterial infection group （ n = 30）, non-bacterial infection group （ n = 32） and mixed infection group （ n = 31 ） according to the examination outcome. The indexes such as gestational weeks of hospitalization,gestation weeks, fetus protection days, body weight of newborns, intrauterine infection rate, pathological positive degree of placenta, delivery pattern, suffocation degree of newborns, complications of newborns and complications of pregnant women were compared in 4 groups. Results： Both bacterial infection and mixed infection groups had shortened gestation weeks and fetus protection days, decreased weight of newborns, and significantly increased pathological positive rate of placenta and intrauterine infection rate （P 〈 0.05 ）. Conclusion ： To enhance the culture and detection of reproductive bacterial in pregnant women with preterm PROM and to perform a timely and targeted therapy may reduce the incidence of preterm birth.
