三维超声成像在甲状腺复杂结节定性诊断的价值与细针穿刺组织活检对比

摘　　要：目的对比三维超声成像与细针穿刺组织活检在甲状腺复杂结节定性诊断上的价值。方法将100例甲状腺复杂性结节患者分成两组,超声组采用三维超声技术对患者结节部位进行扫查,获得超声资料;穿刺组在超声引导下将针穿刺入甲状腺结节,并在不同方位提插5次吸引,取出穿刺物送病理检查,观察两组患者检查后其结节的良性、恶性、可疑恶性及标本不足的情况,统计分析两组的敏感性、特异性和准确性。结果三维超声成像下检查的敏感性为90%,特异性86%,准确性94%,细针穿刺组织活检的敏感性为92%,特异性94%,准确性96%。结论两组敏感性、特异性、准确性等比较,差异不具有统计学意义(P>0.05),三维超声检查结合细针穿刺组织活检具有相互辅助的作用,可以提高甲状腺疾病术前诊断的准确性。Objective Comparison of three-dimensional ultrasound imaging and fine needle aspiration biopsy of thyroid nodules in complex qualitative diagnostic value.Methods 100cases of thyroid complexity nodules were divided into two groups,one group for the ultrasound group,using three-dimensional power Doppler mode for patients with nodular site to sweep,acquired ultrasound information,a group of puncture under ultrasonographic monitoring group,in the needle puncture into the thyroid nodules,and in different directions to insert5draw,take out puncture objects sent for histopathological examination,observation group two patients examined after its nodule benign,malignant,suspicious of malignancy and the specimen is insufficient,the statistical analysis of two groups of sensitivity,specificity and accuracy.Results Three-dimensional ultrasound imaging examination under the sensitivity of 90%,specificity of 86%,accuracy of 94%,fine needle aspiration biopsy of the sensitivity of 92%,specificity of 94%,accuracy of 96%.Conclusion The two groups of sensitivity,specificity,accuracy and show no significant difference（P 〉0.05）,the combination of ultrasound and fine-needle aspiration biopsy with mutually supporting role,can improve the preoperative diagnostic accuracy of thyroid diseases.
