婴幼儿肺炎患者血清IL-8、TNF-α、IgE和外周血EOS水平的临床研究

摘　　要：目的探讨白细胞介素8(IL-8)、肿瘤坏死因子(TNF-α)、IgE、EOS在婴幼儿喘息型肺炎与非喘息型肺炎中的变化及临床意义。方法 72例婴幼儿肺炎患者分为喘息型肺炎40例,非喘息型肺炎32例,与同期健康查体儿30例,用ELISA法测定其血清IL-8、TNF-α、IgE以及外周血EOS水平。同时对喘息型肺炎患儿进行病原学检测。P<0.05有统计学意义。结果两组肺炎患儿血清IL-8、TNF-α水平均显著高于正常对照组(P<0.05),喘息型肺炎组的IL-8、TNF-α、IgE均高于非喘息肺炎组(P<0.05);外周血EOS计数两组间无显著差异(P>0.05);在喘息型肺炎中不同病程和病情的TNF-α水平不同。结论 IL-8、TNF-α在婴幼儿肺炎尤其是喘息型肺炎中起着重要作用;TNF-α水平与喘息型肺炎病程和病情相关;喘息型肺炎IgE较高;RSV和MP是喘息型肺炎的常见病因。Objective Study the change of interleukin 8 （IL-8） , tumor necrosis factor （TNF-α） , IgE, EOS in infants with gasping, non-gasping pneumonia and its clinical significance. Methods 72 cases of infantile with pneumonia, of that 40 cases with gasping pneumonia and 32 cases with non- gasping pneumonia, together with 30 healthy infants checked in the same time were checked for the IL-8, TNF-α, IgE and peripheral blood EOS level with ELISA method. At the same time etiological detection was undergone on infants with gasping pneumonia. P〈0.05 is of statistical significance. Result The IL-8, TNF-α level in the two groups of children with pneumonia were significantly higher than that in the normal control group （P〈0.05） , while in the gasping group was higher than that in the non-gasping group （P〈0.05） , the peripheral blood EOS counts had no significant difference between the two groups （P〉0.05） ; the TNF-α level was different in the group of the gasping pneumonia in different course and condition of disease. Conclusion IL-8, TNF-α levels play an important role in infants with pneumonia particularly those with gasping pneumonia; TNF-α level has association with the course and condition of disease; the IgE is higher by the gasping pneumonia; RSV and MP are the common causes of gasping pneumonia.
