前列腺增生症术后拔除导尿管的方法探讨

摘　　要：目的探讨前列腺增生症(hyperplasia of prostate)术后拔除三腔导尿管(urethral probe)的最佳方法。方法将120例前列腺等离子汽化电切术后留置三腔导尿管的患者随机分为观察组和对照组各60例。观察组于拔管前先将尿液排空再注入37℃0.2%呋喃西林溶液300mL,然后夹管抽空气囊内液体,再注入0.5mL生理盐水后拔除导尿管;对照组将尿液排空,抽空气囊内液体后拔除导尿管。结果二组拔除导尿管时患者疼痛程度、拔除导尿管后出现尿道刺激症状例数、首次排尿的时间比较,差异有显著性意义(P<0.01)。结论前列腺增生症术后拔除三腔导尿管的最佳方法是拔管前排空膀胱注入37℃0.2%呋喃西林溶液300mL,然后夹管抽空气囊内液体再注入0.5mL生理盐水,此法可减轻患者的疼痛不适,避免对尿道黏膜造成机械性损伤,减少尿道刺激症状,促进患者尽早排尿反射,树立正常排尿的信心。Objeetive Discuss the best way to pull out catheter three cavities after the operation of hyperplasia of prostate. Method 120 patients were divided randomly into two groups, one is observation group, the other is comparison group, each group is of 60 patients. For the observation group, first the patient empty urine, then poured 300mL furacilin solution （temp 37℃, concentration 0.2%） , next jammed and evacuated the liquid in pneumathode, finally, pulled out the catheter after poured 0.SmL physiological saline, for the comparison group, empty urine, then pulled out the catheter after evacuated the liquid in pneumathode. Result The differences on several indexs such as the pain degree when catheter removing, the number of patients have the sympton of urethra stimulated after catheter removing, the time of first urination, have a remarkable character meaning （P〈0.01） . Conclusion The best way to pull out catheter three cavities aiter the operation of hyperplasia of prostate is to pour 300mL furacilin solution （temp 37℃, concentration 0.2%） into empty bladder, then jam pneumathode and evacuate the liquid in it, next pour 0.5mL physiological saline, before catheter removing. The method can help to relieve the pain degree, avoid mechanical injury on the urethral mucosa, reduce the sympton of urethra stimulated, speed the patient＇s micturition reflex, build normal micturition confidence.
