摘  要:目的探讨磁共振三维增强在头颈部的成像技术、影响影像质量的因素及其在临床中的应用价值。方法使用德国SIEMENS ESSENZA 1.5 T型磁共振扫描机对105例疑有头颈部动脉疾病患者进行头、颈部动脉三维动态增强磁共振血管成像(contrast-enhanced MR angiography,3D CE-MRA)检查。所有影像用最大强度投影(maxi-mum intensity projection,MIP)进行三维血管重建。结果 105例患者均获得清晰影像,可直观、完整地显示动脉正常结构和疾病分布情况,其中正常19例,异常86例,(分为动脉硬化狭窄闭塞84例,动脉瘤4例,动脉变异7例,烟雾病1例)。结论 3D CE-MRA无创、简便快速,且能了解病变的范围和程度,清楚地显示血管本身病变、病变与血管关系、血管与周围组织关系,在临床预防和治疗动脉疾病中起着重要的作用。Objective To explore three-dimensional contrast-enhanced magnetic resonance angiography(3D CE-MRA) in vascular diseases of head and neck,and the factors influencing image quality,and its clinical application.Methods 3D CE-MRA was performed in 105 patients suspected of head and neck artery disease with 1.5T magnetic resonance scanner(German SIEMENS ESSENZA).All images were processed for 3D vascular reconstruction with maximum intensity projection(MIP).Results Clear images were obtained in all 105 cases,which could demonstrate normal arterial structure and distribution of diseases intuitively and completely.Of them,19 cases of images were normal,86 abnormal;84 cases were found with arteriosclerosis obliteration,3 with variant artery,and 1 with moyamoya disease.Conclusions 3D CE-MRA is a non-invasive and convenient method which can demonstrate the lesions clearly,such as the relationship among vascular lesions,blood vessels and surrounding tissues.Therefore,it is of clinical importance in the control and treatment of arterial diseases.
