潍坊市孕产妇死亡率及相关因素分析

摘　　要：目的研究并分析潍坊市孕产妇死亡情况,探讨孕产妇死亡的主要原因及变化趋势,寻求降低孕产妇死亡的有效途径。方法根据潍坊市所辖县市区妇幼保健院(站)上报的2000年1月—2010年12月孕产妇死亡原始记录及市级孕产妇死亡病例评审情况,对死亡原因进行回顾性分析。结果 2000—2010年潍坊市出生活产数875511例,死亡123例,平均孕产妇死亡率14.05/10万。孕产妇主要死亡原因的前四位分别为产科出血、羊水栓塞、妊娠合并内科疾病、妊娠期高血压疾病。结论加强基层医院产科质量建设和专业技术培训,提高其对高危妊娠的识别、处理水平及转诊及时率;提高孕产妇系统保健率,加强孕产妇高危专案管理;加大健康教育宣传力度,提高孕产妇及家庭成员的自我保健意识可降低孕产妇死亡率。Objective To analyze the mortality of pregnant and lying-in women in Weifang City,and to explore its major causes and trends,so as to seek an effective way to reduce the death of pregnant and lying-in women.Methods A retrospective analysis was carried out for the cause of maternal death on the original medical records of pregnant and lying-in women,and reviewed cases reported by all maternity and child health hospitals（stations） in Weifang City from January 2000 to December 2010.Results During 2000-2010,there was a live birth of 875511 cases,and a death of 123 cases in Weifang City,the average maternal mortality is 14.05/105.The top 4 causes for death are obstetric hemorrhage,amniotic fluid embolism,gestation with medical problems and gestational hypertension.Conclusions The quality construction of obstetrical departments in primary hospitals and professional skills training should be strengthened to improve their ability to identify and manage high risk pregnancy,and to facilitate the referral of patients.The coverage of systematic health care of pregnant and lying-in women should be improved,and special management of pregnant and lying-in women at high risk should be focused on.Voice on health education should be enhanced so as to improve the awareness of self-care among pregnant and lying-in women and their family members,which can reduce maternal mortality.
