摘  要:目的探讨原发性高血压患者血压变异性及血压昼夜节律与颈动脉粥样硬化的关系。方法运用24h动态血压监测(ABPM)对入选的84例高血压患者进行监测,同时根据颈动脉彩超检查结果将其分为硬化组(38例)和非硬化组(46例),对比分析两组动态血压参数。结果①硬化组24 h、白昼、夜间的收缩压(SBP)、脉压(PP)、血压变异性(SSD、DSD)及颈动脉内膜中层厚度(IMT)均明显高于非硬化组,差异具有统计学意义(P<0.05或P<0.01);②硬化组的非杓型血压检出率高于非硬化组,差异有统计学意义(P<0.05)。结论高血压患者血压变异性及血压昼夜节律与颈动脉粥样硬化密切相关。Objective To investigatethe relationship between the variability and circadian rhythms of blood pressure and the carotid artery atherosclerosis in patients with essential hypertension. Methods 24-hour ambulatory blood pressure monitor and carotid artery Doppler ultrasound were performed in the 84 patients with essential hypertension. According to the resuh of the Carotid artery Doppler ultrasound, the 84 patients were divided into two groups:the group with sclerosis (38 cases ) and the group without sclerosis (46 cases). The blood pressure parameters were compared and analyzed in the two groups. Results ①The mean systolic blood pressures, pulse pressures and blood pressure variabilities ( SSD and DSD) of 24 h, daytime and night in the group with sclerosis were much higher than that in the group without sclerosis. The difference was significance between the two groups( P 〈 0.05 or P 〈 0.01 ). ②The detection rate of non-dippers in the sclerosis group is higher than that in the group without sclerosis. The difference was significance between them(P 〈 0.05). Conclusion The variability and circadian rhythms of blood Pressure were closely related with the carotid atherosclerosis.
